Case Manager / Admissions Coordinator (LTC / SNF) – Los Angeles, CA

We have a new opportunity for a Case Manager / Admissions Coordinator in a respected Skilled Nursing Facility in Los Angeles / East Los Angeles, California. As a member of the Nursing Home / LTC / SNF Case Management team, the successful candidate will provide expertise, guidance, advocacy, education, and direction to effectively assist in appropriate admissions and clinical case management.

Case Manager / Admissions Coordinator requirements:

• Long-Term Care / Nursing Home experience required

• Prior Case Management and/or Admissions experience required

• Must speak English and Spanish fluently – Bilingual required

• RN or LVN / LPN preferred, newly licensed nurses encourage to apply

Case Manager / Admissions Coordinator responsibilities include:

• Performing admissions assessment data, MDS, and clinical documentation review

• Developing action plans to improve the Admissions Assessment process as needed

• Performing ongoing Case Management during resident stay

• Maintaining current understanding of state and federal requirements

• Ensuring accurate admissions assessments, timely transmissions, maximum utilization of resources, and program integrity

• Monitoring changes to Medicare / Medicaid programs and adapting case management processes appropriately
