SteppingStone Adult Day Health is San Francisco's largest provider of adult day health care. Each of the organization's four centers provides an array of services, including nursing and personal care; medical social work services; meals and nutritional services; medication management; physical, occupational, and speech therapies; and social and recreational activities; as well as round trip transportation via wheelchair accessible vans. Presently, SteppingStone serves over 300 ethnically diverse seniors and adults with disabilities each month.


Outreach Coordinator/Member Services


The Outreach Coordinator/Member Services incumbent is responsible for increasing the organization’s presence in the community and will build relationships with individuals and other organizations in support of client/participant recruitment and placement within SteppingStone’s programs. The full-time position works under the supervision of the Executive Director, and will liaison with Program Directors and Social Workers as clients/participants transition into the organization.


1. Identify potential client/participant needs and determines their potential eligibility for adult day health care (ADHC) center programs as established by Title XXII requirements and CBAS eligibility criteria.

2. Provides outreach services for the ADHC program by developing and maintaining relationships and personal contacts with referral sources (i.e. local communities, organizations).

3. Becomes familiar with local communities and organizations, and establish relationships with case managers, social workers, discharge personnel at hospitals that generate potential qualifying referrals.

4. Create materials (i.e. brochures, mailings, promotional materials) with the Development Director to facilitate outreach efforts.

5. Conduct presentations that generate qualifying referrals to organizations, staff meetings, community meetings, etc.

6. Perform program tours, when available, of SteppingStone centers and explain the program to potential participants, family members and caregivers.

7. Processes referrals to the ADHC program by collaboration with Social Worker Assistants and collecting client data, completing intake forms and related documentation, and explaining fee structure.

8. Acts as an integral member of the Multidisciplinary Team group by facilitating client attendance and participation in accordance with person centered care best practices.

9. Offers additional information and referrals to other resources as necessary.

10. Attends staff meetings as appropriate, as well as semi-weekly Management Team Member meetings.

11. Other related duties as assigned.


1. Bachelor’s degree from an accredited college, preferably in Public Relations, Marketing/Advertising, Public Administration, or Social Work.

2. 2 to 5 years of increasingly responsible relevant work experience.

3. Demonstrable aptitude working with older adults and individuals with disabilities.

4. Bilingualism is an asset, and Cantonese language conversational skills strongly preferred.

5. Excellent inter-personal skills.

6. Excellent organizational, written, and oral communication skills.

7. Project management experience.

8. Detail oriented and committed to meeting deadlines and objectives.

9. Physically capable both of performing the required duties and of assisting frail elderly and disabled adults, as necessary.
