Job Duties, including but not limited to:

-Perform a full range of medical services within the scope of a Pharmacist to all patients and eligible beneficiaries

-Serve as liaison between the Department of Pharmacy Medical Staff, -Department of Nursing and patients for pharmaceutical matters, and as an expert on pharmaceuticals, pharmacokinetics, and pharmacotherapeutics.

-Serve as therapeutic consultant, attend medical rounds and conferences with the physicians to provide information used in evaluation and implementation of optimal medication plans for inpatients and outpatients

-Analyze selected medication programs for accuracy, incompatibilities, contraindications, and appropriate dosages.

-Monitor patient medication therapy for effectiveness, side effects and adverse reactions.

-Develops, orders, and adjusts individualized medication regimens per protocol and consent of physician

-Counsels patient on the use, proper method for administration of medications, the common potential side effects from medication usage, and precautions and other general information

-Assist the medical staff in designing, coordinating and accomplishing an effective medication utilization evaluation (MUE) program to promote and sustain optimal therapeutic effectiveness and reduce waste.

-Perform educational activities: support and provide routine and complex drug information to patients, physicians and other staff through literature, analysis and research. Provide lectures for continuing education to pharmacy, nursing and medical staff.

-Responsible for pharmacy related operational policies of the clinical area and functional direction of pharmacy staff members providing tasks related to clinical service, e.g., preparing discharge medications, bedside medication, medication labeling, and adjusting specific intravenous drug preparations

Minimum Qualifications:


-At minimum must have a Post Baccalaureate, entry level Doctorate of Pharmacy (PHARMD) degree or a master’s of science in pharmacy from a clinically oriented program OR board certification in one or more of the Pharmacy specialties recognized by the board of Pharmaceutical Specialties.

-Must have completed a Clinical Pharmacy Residency or fellowship accredited by the American Society of Health System Pharmacist or American College of Clinical Pharmacy


-Minimum of one (1) year of experience as an active Pharmacist in the last three (3) years.


- National Background Investigation

- Must be authorized to work in the US

- Must have reliable transportation to and from work site

License/Certification and Training:

-Maintain a valid unrestricted current medical license in one of the 50 United States, District of Columbia, Puerto Rico, Guam or U.S. Virgin Islands with ability to obtain clinical privileges

-Basic Cardiac Life Support (BCLS) through American Heart Association; No other program accepted

-National Practitioner Identifier

-Graduates of foreign medical schools must possess both a valid license and certification by the Educational Commission for Foreign Medical Graduates (ECFMG)
