Under direction of the West Texas Regional Coordinator, the Lubbock Clinic Coordinator is responsible for the daily operations of case management at the Epilepsy Foundation Texas and education/outreach in West Texas.

Essential Duties/ Responsibilities

Clinic Services:

•Work with doctors/staff in coordinating patient care and medical needs

•Schedule, attend and assist with epilepsy clinics, manage appointment scheduling

•Maintain DSHS paperwork and follow procedures including determining patient eligibility for DSHS clinics

•Maintain EFTX documentation, records and reports

•Provide education and direct referrals to local resources for patients

•Maintain all patient assistance programs and assure patients medication accessibility

•Request and send out medical records

•Reach out to former and potential clientele

•Assess client emotional, physical, and social needs

•Assist in problem solving

•Locate and coordinate services, assist with forms

•Maintain written reports/ logs as required; document client’s activities

•Make referrals to other community agencies as needed

•Arrange for patient transportation assistance as needed

Community Education/Outreach:

•Maintain effective working relationships with healthcare agencies, community organizations, staff, and the general public

•Work to increase public awareness of epilepsy and services provided by EFTX

•Promote, arrange and conduct epilepsy education events and conferences including- coordination with speakers, vendors, etc.

•Provide outreach and education across territory as needed to physicians, schools, daycares, nonprofits and other agencies to meet education numbers as required

•Attend and assists with West Texas events

•Nights and weekends may be required for events and educational programming, etc.

Knowledge, Skills, and Abilities

•Skill in using Microsoft office programs (Word, Excel, Power-point)

•Ability to organize and prioritize work

•Ability to communicate effectively orally and in writing

•Knowledge of community resources

Required qualifications

•Bachelor’s Degree (preferably in social work or other social service/healthcare field)

•Ability to work independently with limited supervision

•Ability to travel as needed. Must be able to provide own reliable transportation, a valid driver’s license and personal auto insurance.
