Rapidly growing company is seeking a professional, dependable LPN with a passion for caregiving.

Job Description: As a Licensed Practical Nurse, you will work with patients in their homes and provide them with an array of services.

• Providing hourly patient care in the patient's home

• Assisting in the development, implementation, evaluation and revision of plans of care

• Implementing the nursing plan of care

• Documenting and communicating patient care needs and assessments

• Providing teaching and guidance to patients, families and the community

• Accepts patient care reports and patient assignments from the Primary Nurse and/or Case Manager for all patients assigned

• Provides frequent communication to the Case Manager, physicians, other team members, etc., and the patient/family in efforts to coordinate changes in patient care and assures appropriate follow-up and supporting documentation is done

• Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes

• Maintain the highest standards of professional conduct by delivering care in a manner that protects and preserves the patients dignity, rights, values, beliefs, privacy and autonomy

• Complete a clinical note for each visit within required timeframes, capturing physical assessment, home environment, medication changes and any skilled interventions provided during the course of the visit as well as any care coordination with healthcare team. This note will be incorporated into the patient's record.

Job Requirements: Your nursing skills have never been more appreciated! This job requires a person who can work independently, yet never forget that continuous care requires a team effort. Additional requirements include:

• You must have a valid State of Tennessee nursing license or a license from a compact state.

• At least 1 year of nursing experience in a pediatric or geriatric setting

• Ability to cope with the emotional needs of patients and their families

• Ability to communicate effectively, both verbally and in writing

• Respecting the patient's privacy and maintaining confidentiality

• Current Tennessee driver's license and automobile liability insurance

• CPR Certification

• You must have a current TB skin test

• You must be able to satisfy employee screening results (background check, drug testing, motor vehicle report, licensure verification, abuse registry, OIG excluded individuals)

• Working knowledge of the Private Duty care program and previous experience in Private Duty preferred

• Trach, Ventilator, G-Tube/Peg-Tube experience preferred
