SUMMARY: The RN is a clinical practitioner who assesses, plans, coordinates and implements patient care specific to the pediatric population we serve. He/She ensures that compassionate, engaged care meets or exceeds quality standards. Care is provided in an efficient and safe manner consistent with the Maryland Board of Nursing licensure and Hospice of the Chesapeake (HOC) standard of care. Utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care, cultural, ethnic, spiritual, and educational needs of the patient/family/significant other to develop, implement and revise the plan of care. The RN recognizes the legal responsibilities as specified by law and abides within those responsibilities.

HOURS: Monday – Friday 8 a.m. – 4:30 p.m.

DUTIES & RESPONSIBILITIES:

•Conducts focused Skilled Nursing Visit, wherever a patient resides, to address a specific concern.

•Evaluates, treats, reassesses and documents pain as appropriate to patient condition or need and uses appropriate pain scale.

•Significant status changes are identified, communicated, responded to and documented.

•Performs liaison/consent visits as scheduled to provide information regarding HOC services.

•Correctly identifies, communicates and collaborates on patient problems/priorities/issues based upon assessment data, establishes measurable patient goals and communicates initial/ongoing plan of care appropriately to interdisciplinary team. Recognizes changes in clinical status and reports them to the appropriate individual.

QUALIFICATIONS:

Possess a current Maryland Board of Nursing (RN) license and thorough knowledge of nursing principles. Bachelors of Nursing and CHPN preferred.

SKILLS: Excellent oral, written and interpersonal communication skills. Demonstrated emotional maturity and ability to cope and work effectively in stressful situations. Demonstrated ability to work independently with minimal supervision. Demonstrated ability to work effectively on an inter-disciplinary team. Excellent customer service philosophy and skills required. Demonstrated organizational and time management skills. Proficiency in computer skills; All Scripts preferred. Demonstrated understanding of and concurrence with the hospice philosophy of palliative care.

LOCATION: This is a field position, servicing pediatric patients in Prince George's County and Anne Arundel County.
