We seek PRN Field Care/Case Managers: complete assessment, creates care plans, and follows up in-home visits. Need 2 Field Care/Case Managers for locations: North to Central Houston and Southwest Houston

and nearby surrounding areas. 1 Field Care/Case Manager for North to Central Houston and 1 Field Care/Case Manager for Southwest Houston and surrounding areas.

Work on interdisciplinary care team, have full administrative support and you control your schedule.

MINIMUM REQUIREMENTS:

• Min. 2 years of case/care management experience (can have at least 3 yrs community based or in-home health visit experience in lieu of case/care mgmt experience)

• Min.3 years LVN, RN, or Clinical/Med Social Worker confirmed work experience under license

• Min. 2 years working with elderly, chronically ill or functionally challenged populations

• Strong organizational skills; time management, consistent follow-up, attention to detail

• Motivational interviewing and coaching skills

• Computer and internet navigation skills

• Excellent written and verbal communication skills

• CAN NOT have any past or present disciplinary actions on license

• Must be able to work unsupervised, have good time management skills,

• Willing to travel at least 30 miles radius of base location

• Currently working in the field preferred

• Per Diem

Additionally, must have access to reliable vehicle with driver’s license and insurance. This position offers a flexible schedule but the candidate must also have a flexible schedule (between 10am and 5pm).

English-Spanish bi-lingual a plus.
