PHC Assistant Coordinator – Home Health – Austin TX

$11 to $13 per hour – Full-time

The PHC Assistant Coordinator is organizing patient care activities and sharing information among all of the participants concerned with a patient's care to achieve safer and more effective care. The PHC Coordinator is expected to know the patient's needs and preferences ahead of time and communicated at the right time to the right people, and that this information is used to provide safe, appropriate, and effective care to the patient.

Job Responsibilities and Roles

•General Office Duties and Perform admissions and program orientation

•Conduct attendant orientation and training

•Monitor and analyze Quality Improvement issues and direct field staff in regards to agency policy and procedures.

•Perform Home Supervisory Visits periodically and as needed.

•Perform chart audits and resolve billing/claims issues

•Medical Record Filing

•Knowledge of Electronic Visit Verification

•Perform EVV maintenance

•Process and submit accurate required paperwork by due dates to State Organizations per policy, procedure and form instructions/guidelines.

•Demonstrate ability to keep track of clients Hospital admissions/dismissals, clients out of service area, client suspensions, client service breaks, and prepare 2067’s with necessary patient information.

•Oversee and Coordinate all Primary Home Care/PAS Services

•Attendant recruiting and management

Job Qualifications

High School Graduate or GED. Associates Degree preferred

Good interpersonal and communication skills. Bilingual in Spanish a plus, but not required.

Conditions for Employment

•1+ year experience working in home healthcare or medical office setting. No Job hoppers.

•Experience with EVV and working knowledge of VESTA software a plus

•Pass a pre-employment criminal background check.

•Proof of auto insurance and valid driver’s license.

•Must have reliable Transportation

•Must be a team player

•Must be ready to work Full-Time

•Must have good computer skills, to include basic computer software like Microsoft office (Word and Excel)
