We have a new employment opportunity for a RNAC / MDS Coordinator with a well-respected Long Term Care Facility located in the Chico, California area.

This position requires a detail oriented nurse with substantial MDS 3.0 experience and a desire to ensure quality in medical care for long term care residents. The chosen candidate will be a Nurse with a current license in good standing & current CPR certification. RN required.

The MDS Coordinator will provide guidance to facility employees pertaining to MDS processes and their correlation to resident care and reimbursement. This person must carefully follow revisions to MDS/PPS requirements and sure compliance with current regulations.

MDS Coordinator responsibilities include comprehensive assessment for all residents:

• Physical needs

• Clinical status / dx

• Medications

• Cognitive patterns

• Nutritional status

• Skin condition

• Psycho-social functioning

• Life care wishes

Additional responsibilities include;

• Auditing and analyzing MDS data for accuracy.

• Efficiently resolving any issues.

• Providing feedback to corporate leadership on the status of clinical and financial reimbursement.

• Identifying areas for improvement, formulating an action plan, and following through to ensure progress.
