I am a dedicated employee as you can see by how many years I worked at St. Clare’s Hospital. I am not only willing to learn about my job duties, but also the duties of others so that I may help others as much as possible. I work well with other people including patients and other workers.

Work History 08/16/10 to present Florence correctional facility I work anywhere between 36hrs and 48 hrs a week. I work 12 hrs shifts. I am responsible for triage, sick call, distributing medication, ekgs, phlebotomy, emergency response team, EMR documentation. Scheduling nurse visits and/or Provider appointments. Preparing clearances for those inmates that will be leaving the facility . Making sure that everyone is up to date on their PPD.

5/3/10 to 08/01/10 Medstaff inc travel agency VA CLINIC STOCKTON CA This was a three month travel assignment. I worked 40 hrs a week. My duties included. Rooming pts, taking their vitals, reviewing their chart to make sure that they were up to date on any immunizations or flu and pneumonia shots. Dressing changes, ekgs and preparing them for the provider.

11/14/2009-4/21/10 ERIE COUNTY NURSING HOME. I worked as a floor nurse on the night shift and was responsible for anywhere from 50 to 55 residents and 3 to 4 CNA’s. I administered medications, did treatments, phlebotomy, fingersticks, insulin injections, tube feeds including bolus and electronic pumps. I charted on the EMR system. Prepared residents who were leaving for appoitments or with family for the day. I was responsible in overseeing the work of the CNA, to make sure that all protocols were being followed and that every resident was being taken care of properly. Handling all emergencies and initiating CPR if necessary. Respiratory care.

5/21/07 – 9/21/2009 Lifetime Health Medical Group, Licensed Practical Nurse.

My duties are as follows: Patient screening, EKG’s, Fingersticks, insulin.

Immunizations, blood draws and respiratory treatments on both Pediatric and Geriatric populations. I worked 40 hrs a week. Charting on the EMR. Vital signs, measurements including height and weight. Ear lavage. Pt teaching.

I work on Unit 3, and my supervisor is Nancy Vierra, RN. I worked 40hrs a week on the day shift 7-3. My duties included administration of medication, treatments, ekgs, overseeing the work of the CNA’s who were assigned to me to make sure every resident received the proper care. Resident care and transportation, feedings either by mouth or via g-tube. Charting on the EMR. Taking orders from the provider and transcribing them. New admissions and discharges. Respiratory care and range of motion exercises. Talking with families. Fingersticks and Insulin and flu shot administration.

3/05 – 6/05 Erie County Medical Center, Licensed Practical Nurse

I worked on 10 South, Medical / Surgical Geriatrics.

My supervisor was Patricia Kiblin. I worked the night shift 11-7 full-time 40 hrs a week. My duties included. Pt care, treatments, resp care, phlebotomy, iv initiation, tube feeds including electronic or bolus, fingersticks and insulin, ear lavages, CBI’s, medication administration including iv antibiotic piggy backing. I worked here for a short period of time because I needed a day time job do to home circumstances. Charting on the EMR.

1981 – 2004 I started at St. Clare’s Hospital as an orderly in 1981, until 1986.

In 1986 I became a Nursing Assistant until 1992. In 1992 is when

I went to school for becoming a Licensed Practical Nurse. I did

Patient Care on a busy Medical/Surgical floor that specialized in

Orthopedics and Hospice. I worked completely with the scope of

my practice. I performed suction on patients, monitored IV lines,

run CBI’s (Complete Bladder Irrigation) and changed dressings as

ordered. I administered medications, PO, IV, IM and PR. We did

work on computers as in placing orders and retrieving various

reports and lab values. I charted on patient’s conditions, including

assessments. I also do phlebotomy, EKG’s and preceptoring. I can set up and take down traction equipment. I also have experience in working the OR as a technician.

1992 (8 months)Guilderland Center Nursing Home, Licensed Practical Nurse.

My duties included administering medications, charting, resident

Care, dressing changes and Charge Nurse Duties when needed.

Education

1990 – 1991 LPN, BOCES School of Nursing. I presently have many credits

towards getting my license to become an RN.
