OBJECTIVE: I am seeking a position as a nurse practitioner. I have strong clinical nursing background.

Work Experiences

1. Family Nurse Practitioner at The Little Clinic on 4656 Cemetery rd, Hilliard, Ohio, 43026. I have been providing primary care to patients from age 18 months and above since September 13, 2010 to present time.

2. Family Nurse Practitioner at East Main Family Medicine on 881 East Main St, Columbus, Ohio, 43205: I have been working part time at this clinic since July of 2010, and have been providing care for individuals from birth to late adulthood.

3. Online clinical instructor at Chamberlain College of Nursing on 1350 Alum Creek dr, Columbus, Ohio, 43209. I have been working at this college since July of 2009, and I have also taught in classroom setting and in different clinical setting as partime and fulltime instructor.

4. Staff Nurse at Ohio Health-Doctor’s West Hospital from July 2001 to April of 2010: Worked in different units of hospital including medical surgical telemetry, emergency room, intensive care units, and step-downs. Worked independently mostly night shifts and often had to assume the role of a charge nurse and be a resource person for others including new interns and other nursing colleagues.

5. Weekend Nursing Supervisor at Sanctuary at Tuttle Crossing, Dublin, Ohio from spring of 2005 until fall of 2005- I worked at this facility during the day, on weekends as a nursing supervisor and tackled different patient care and staff issues.

6. Staff Nurse at Select Specialty Hospital, Columbus, Ohio from August 2000 until April of 2004- At this facility I took care of patients with complex rehabilitation issues including patients suffering from traumatic injuries to heart attacks, patients with extensive wounds, and mobility problems.

7. Staff Nurse at OSU-East Hospital from December of 1991 to July of 2002- At this facility I worked about one year on a medical-surgical floor and 4 years on the oncology floor (taking care of patients of Dr. Rupert/Dr. Ghani, the hematology oncologist) the and then worked the remaining 5 years in the emergency room.

8. Staff nurse at Medstaff nursing agency: Worked about one year @ James Cancer Hospital thru this nursing agency on ENT oncology and OB/GYN-abdominal oncology units

9. Staff Nurse at OSU hospital form June of 1991 to December of 1991-I worked as a new graduate-nurse in the critical care internship for about 6 months and rotated through all their critical care units.

Experience as a Student Nurse Practitioner

1. Followed physician as a general practitioner in an urgent care setting for a total of 80 clinical hours

2. Followed a family practice physician in a family care clinic for 120 clinical hours for my pediatric rotation

3. Followed and shadowed a family care physician for my women’s health rotation for a total of 120 clinical hours

4. Followed 2 different nurse practitioners that managed care for patients in extended care facilities for the total of 160 clinical hours

5. Followed 2 nurse practitioners, one that managed care of patients in extended care facilities and the other nurse practitioner who worked in a drug rehab clinic for a total of 160 hours.
