OBJECTIVE:

To provide all aspects of Pharmacy Services under the guidelines of Texas State Board of Pharmacy to Community ,Long Term Care facilities, Surgical Centers and Hospitals.

SUMMERY:

Conducting all duties of day to day operations of a Community Pharmacy. Perform DRR on all prescriptions received. Review patient medical history for any allergic contraindications, safe dosage and correct indication. Advice on proper med usage ensuring patients know the correct dosage of meds taken, correct way to measure dosage and how often it is taken. Counsel and advice patients on wide array of health topics. Provide excellent Customer service to retain and increase business. Operations management responsible for overseeing other pharmacy staff. Ordering and stocking health, beauty, and medical supplies.

Perform on-site Consultation in a Long Term care facility. Conduct a full Drug Regimen Review .Advice Physicians and Nursing personnel on drug interaction, correct dosage, incompatibilities, side effects and instructions for drug administration Monitor medication errors rates and make recommendations to enhance risk management. Participate in monthly QA meetings. Play an active role in the development and implementation of Policies and Procedures under the guidelines of State Regulations.

Clinical Staff Pharmacist at Surgical Hospital, evaluating and interpreting med orders, including identify, prevent, assess, and resolve medication-related problems to ensure a safe and optimal patient outcome. Promote, through criteria based protocols. clinically rational drug therapy and sound pharmaceutical care.

Pharmacist for the past fifteen years. employed with various retail chain stores- Walgreens, Kroger, Eckerds.

LTC facilities - Omnicare, Pharmerica, LTC Pharmacy.
