ü Record patient data prior to each physician visit, including but not limited to vital signs, height, weight, allergies, medication changes.

ü Initiated a patient education plan as prescribed by physician including patient and family instruction.

ü Revised plan in consultation with physician based on ongoing assessments and as required by policy/regulation.

ü Analyzed any changes in a patient’s condition. Identified symptoms and severity while alerting the physician and Charge Nurse promptly.
