Director, Case Management – QueensCare Family Clinics, Los Angeles, CA.
• Responsible for adapting and implementing the LA County espoused Chronic Care Model for disease management.
• Developed and implemented curricula for chronic diseases such as diabetes, hypertension, obesity and asthma with focus on nutrition, physical activity, medications and possible complications with input from physician champions and clinical pharmacists.
• Supervises case managers who perform one on one as well as group counseling for patients who have chronic diseases such as diabetes, hypertension and asthma . The same is done for those who are at high risk because of high cholesterol, are overweight, or have family history positive for these risks.
• Individual or group diabetic teaching – includes Insulin administration and proper use of glucometers.
• In charge of the Cope program whose objective is to ensure appropriate utilization of emergency department and acute admission.
• Regularly reviewed all managed care members who have visited the ER.
• Coordinator of a state-funded program called WISEWOMAN, the objective of which is to prevent cardiovascular diseases on Latina women ages 40-64 yrs old
• In charge of emergency preparedness with good knowledge of what to do when a natural or man-made disaster happens. In this capacity, most community clinics in Los Angeles participate in the statewide drills. My role is to facilitate a tabletop discussion among key members of the Incident Command System
• Started a Tobacco Cessation Program for patients who are motivated to quit smoking

Director, Case Management – Sherman Oaks Hospital
Responsible for the Case Management and Social Services department of a 150 bed facility .
Wrote, reviewed and revised the Case Management plan, departmental budget, policies and procedures, quality indicators for performance improvement. Reported findings and outcomes to hospital committees. Co-chaired the Peer Review Committee. For this monthly meeting, I was responsible for investigating and presenting cases that resulted in unexpected outcomes.
I was also given the task of calling an emergency subgroup committee for sentinel events.
All reviews and decisions made by the committee were entered into a peer review data base.
1995 –1997
Manager, Case Management – Prudential Health Care of Southern Ca.
Responsible for supervision and oversight of the catastrophic case management department for Prudential members in Los Angeles and Orange Counties. Performed concurrent telephone and on site reviews. Was trained, and in turn, trained the case managers and support staff on the Prudential case management computer information system.

Responsible for supervision and oversight of the Case Management Department of a 400+ full service hospital facility, and later also became responsible for the Northridge -Sherman Way Case Management departments during the transition from Unihealth to CHW.
