The primary goal of a nurse case manager is to ensure that every patient in their care receives exceptional care for

your facility, agency, or company. To accomplish this, nurse case managers assess new patients by gathering information, reviewing diagnoses, and analyzing medical test results. Case Managers develop complete plans of care to address the needs of patients. They educate patients about their care plan and health care options and help them make key decisions.
