Develop, implement, and review healthcare plans for patients, including conducting initial patient assessments within 24 hours of admission while overseeing a 25-bed Medical Surgical Unit. Empower individuals to maintain medication and treatment regimen compliance by providing ongoing instructions, guidance, and support. Spearhead the development and implementation of care plans for patients to address their individual needs. Formulate Home Health Orders, Durable Medical Equipment Orders, and DMA6 (Physicians Recommendation Concerning Nursing Facility Care) documents through EPIC EMR.

Assesses progress toward goals and identifies barriers to meeting goals. Prepares and maintains appropriate documentation of patient care and progress within the EMR. Closes cases in accordance with defined case closure procedure in a timely manner and in accordance with established guidelines.

Advocate in the patients’ best interest for necessary funding, treatment alternatives, timelines, and coordination of care, with frequent evaluations of progress and goals. Continues to identify our community and caregiver resources to ensure continuity of care during and after completion of the care management plan.

Responsible for designated unit-based patient cases and plans effectively to meet patient needs, manage the length of stay via monitoring and coordination of care pathways and promote efficient utilization of resources.

Address all social services/healthcare issues from the nursing staff, or physician while acting as a liaison between the patient, family, outside agency, physician, and the hospital.

Track utilization of professional services, service delays, discharge delays, and reports as necessary. Provides collaboration with the attending physician to work through the delays.

Works with Physician to establish the appropriate admission status for billing. Ensure all aspects of the process are addressed from a CMS compliance standpoint.
