Fulltime Physical Therapist: Montgomery Nursing and Rehabilitation center, New York Dec 2018 to present.

Working as a Physical Therapist in Subacute care setting and long-term care.

Perform initial physical therapy evaluation, establish patient-oriented goal setting and plan of care.

Monitor progress, document daily treatment notes, periodic progress notes, recertification notes and complete discharge summary at the end of skilled therapy.

Supervise physical therapist assistants, rehab tech staffs and update sigma care to ensure safe patient handling.
