Career Background:

Having completed a Full Rigging Apprenticeship as a Rigger in the Ship Repair Marine environment this enables me to Construct, test, inspect and certify all kinds of Heavy Rigging/Lifting equipment. My Career has progressed along the lines from working both On and Offshore to also Overseas, where I have come accustom to coping in both a Supervisor and Superintendent position, and Head of Maintenance Services, Rigging and Scaffolding Inspection, Heavy Lifting, Rigging, Crane and Transport Operations along with Scaffolding work, and the Various types of work in the field of fabrication and construction for Onshore Fabrication and Offshore Hook-Up work. I have been involved in various projects with the use of Heavy Duty Crawler Cranes for Multi-Crane Lifts for Onshore roll ups and for Offshore Jackets and Topside platform module’s which have weighed up to 4000 tones, and structures Lifting, skidding, jacking and packing the Modules to lift the Modules to the require high to be moved and placed on the Final foundations strand jack work the weighing and transporting for various contractors and projects. Focal point for the co-ordination of all lifting and rigging services offered to the client company such as but not limited to: LOLER, Development of lifting plans throughout the terminal.

My primary role of the Site Lifting Coordinator is to provide continuous review of lifting practices to ensure compliance with this SSOW.

As the Site Lifting Coordinator has a coordination role, and is responsible for ensuring that a regime is established and maintained on the site such that lifting operations are carried out safely. This will require:

The authorization of all lifting plans and risk assessments for all categories of lifts, supported by the Lifting Technical Authority and Mechanical Handling Contractor's (MHC) technical support engineer as appropriate This includes ensuring that it is clear who is in charge of specific lifting operation.

Ensuring that all personnel involved with any lifting operations on the site have proper training and are assessed as competent to SSOW standards and that a register and documentation of such personnel is maintained on the installation I site. This includes resident site personnel and any ad hoc personnel. The person must also have an in depth knowledge of the relevant legislation, regulations, standards, company procedures, health and safety aspects, and industry best practice.

Maintain the register of crane operators and cherry picker operators authorized to perform lifting operations

Manage and control the rigging and lifting loft inventory

Maintain the register of generic and specific site lifting plans and risk assessments

Have the responsibility for all site lifting activities

Authorize lifting plans and risk assessments for all lifts

It is essential to ensure that a competent person visits and inspects the work site at defined intervals to verify that the conditions detailed in the permit have not been compromised and that the work is getting carried out in a safe manner.

Have an understanding of the Company Safety Policy and the responsibilities of each grade of personnel. Ensure that the policy is brought to the attention of all employees under your control and ensure that work is carried out in accordance with its requirements.

All employees working directly under my charge are aware of their responsibilities with regard to the Company Safety Policy and their obligations for safe working.

Have a knowledge of regulations, standards, codes of practice and company procedures applicable to work which you control and ensure that they are complied with.

Discuss working methods with Appropriate Managers and Charge hands etc. and where necessary consult the Company Technical/Safety personnel to determine adequate safe systems of work.

Where these exist but are beyond the Company's control, before allowing work to proceed, obtain documented evidence to ensure that the Client or the controller of the premises has carried out all necessary precautions.

All work that is carried out on the Shore Base is carried out in a safe and correct manner and in compliance with relevant risk assessments, method statements, permits to work and any specific site safety requirements.

When personnel under my supervision, also receive and use appropriate personal protective equipment and other safety equipment and ensure that it is used.

All tools, plant and equipment, whether hired or purchased, are safe and suitable for the work. Arrange for regular servicing and maintenance in accordance with Company procedures and where applicable statutory requirements.

Ensure that no defective tools, plant or equipment are used and that all defects are rectified promptly. Also ensure that no tools plant or equipment are modified in anyway, except when carried out in an approved and controlled manner.

Ensure that all lifting appliances, lifting gear and other equipment has undergone statutory tests and examinations and that all mandatory documentation is in order before use.

When the operation of stacking and placing of materials and supplies so as to avoid double handling. Ensure that all materials are stacked in a neat and safe manner and where necessary provide for the protection of weather or temperature sensitive materials. Also ensure that only materials which are fully serviceable are issued for use.

All unsafe acts or unsafe conditions (near misses) are reported by Operatives or HSE Supervision are recorded via the safety Observation system and appropriately actioned in an attempt to prevent future incidents.

Identify any requirements for the training of staff and operatives under your charge and in consultation with the relevant Unit Manager, make sure that training is given when and where required to all personnel Ladies and Gentlemen for Banksmen/Slingers This would also include induction training for new employees (or when transferring from another site). It also includes both GYSBI and Exxon "Toolbox Talks" which should be held on all sites and premises on a regular basis. Ensure that records of all training and inductions are kept in accordance with Company procedures via the training coordinator.

Ensure that no employees are allowed to conduct any task or use any equipment until they have received adequate training, information, instruction and supervision to enable them to do it safely.

Ensure that competency assessments are carried out for all employees under your control, in line with Company procedures, and that they are competent to safely carry out their duties.

Set a good personal example at all times in all respects including timekeeping, wearing of PPE and adherence to GYSBI working procedures.

All national employees involved in rigging and lifting duties are provided with support and guidance to complete their competence assessment portfolios to gain he required levels of training also act as assessor and expert witness whenever required

My position with JAD Construction Company is to overlook the Lifting/Rigging and Transport actives in all of their Bases in Warri Nigeria, I am mainly based at the Base 4 Yard, where we have a various types of Crawler and Mobile Cranes and SPMT’s trailers, at the moment we have been doing an overhaul on 4 no Liftboats/Jack-Up Barges, which includes the removal of the Full Leg Sections from each barges to the Fabrication Shop for

The renewal of some sections of the legs also repairs to the Jack-up Pads and the overhaul of the racking/cogs systems prior to been re-installed back on the barges.

The removal and re-install of Deck Cranes after blasting/painting and other repairs, also the re-testing of all the cranes, which involved a third party Inspection Company.

Other types of work we are involved in is to hire equipment and lifting services for Chevron and Shell to their various projects located around Nigeria.

I would report to the General Manager of the Company, the main functions of my role would be to review and validate the lifting procedures according to company / project specification ensure all works are carried out following approved procedures

Supervise and coordinate all activities with local personnel and equipment engaged in lifting operation in close coordination with HSE / QA / QC and construction superintendent

In my position as Yard Supervisor is to organize and responsible for supporting the Management Team and supervisor in the yard team to ensure the efficient running of the yard function, supporting the smooth operation of the yard and to ensure that all equipment is loaded and un-loaded are delivered on time and with minimum disruption in line with standard Company operating practices and HSE requirements.

Responsibility for supervising the yard team, for the loading and un-loading with the use of a cc4800 Crawler Crane(800tons Capacity) of Heavy Duty Umbilical Tubing, Wire Rope, Offshore Wind Industry for the Offshore Industry worldwide and also the support of sub-contractors in the Yard including all Rigging Personnel, Heavy Duty and Mobile 4800CC

Crane Operators along with in line drivers, fork-lift operators ensuring the yard is managed effectively, including ensuring administration, coordination and movements of vehicles meets customer and internal requirements at all time.

To ensure the smooth day-to-day operations of the departments, both the Rigging and Scaffolding Dept.to deal with any problems or incidents in a timely manner and within the required procedure guidelines, escalating issues to the appropriate manager Plan in advance to support vehicle arrivals in terms of facility available, productivity of labour and equipment, and where appropriate support the team with ongoing process and movement of inspected vehicles, to ensure the efficient and effective flow of vehicles around the site ensure accurate communication is maintained with Vendor Support colleagues to minimize the time delays in getting a vehicle allocated.

To proactively manage the development and performance of all team members, ensuring that all mandatory HSE requirements are fulfilled and all team members are trained and developed to maximize performance taking appropriate action where standards are not met always make sure that the yard is presentable so that it meets HSE requirements and gives a good impression to our customers.

Hold TBT and Pre-Work Safety Talks and organize any permits along with Yard Induction Safety Courses for various sub-contractor working on site which is required prior to any loading and loading of equipment, make sure that all the rigging and lifting equipment is Inspected/ Color Coded and regularly updated as per L.O.L.E.R. and P.U.W.E.R. regulations

Position: Field Supervisor Lifting Hoisting/ Rigging Specialist

Project: Phase 2 and 3 Malampaya Deep-water Gas to Power Project

My duties were to assist in the Yard Readiness review team in the Lifting and Transport operations making sure that that all rigging and lifting equipment meet with the SHELL Lifting and Hoisting Code of Practice to help the Keppel Shipyard integrate their Ship repair Lifting Procedures in line with Shell Asia Lifting and Hoisting Procedures along with the up grading of the Shipyard equipment prior to the contract starting.

Working alongside the Keppel Engineering Team and HSE Department to promote safe handling & lifting Operation referring to HSE policy and international standard Maintain HSE standards and QA/QC Standards. During the Heavy Lifting Program, preparation of Lifting Plans/Studies, Method Statements Work Procedure Act as a technical advisor of handling and lifting service along with Managing handling and lifting cargo personnel Heavy Lifting check Lists are completed Job Hazard Analysis / RAMS/TBT, Take 5’s Pre-Lifting meetings Prior to any Lifting Operations taking place.

Work includes field inspections and the physical ability to conduct field inspections which require the following Activities: extensive walking in and around a construction site, climbing ladders, climbing scaffolding, and working on elevated platforms, working on uneven surfaces and working in small or confined spaces. Where some lifting may be rrequired.

Must have the ability to walk into and out of the project (enter/egress). Must be able to use appropriate standard issued Personal Protection Equipment (PPE) required for each assigned task (i.e. harness, life vest, lift Buckets, etc.)

Assist in preparing Erection sequence and Testing Method procedure for the erection of new 1500 ton overhead Dockside Gantry Crane to assist in the construction of the 3 No Main Barge Deck sections of 600 tons to over 1000 tons and 5 No Module Sections which are between 500 tons and up to 600 tons on the Malampaya 3 Project the crane was also using for installing 4no x 450 ton 60 mtrs. Upper Leg Section onto 4 No’s Base Footings of 465 tons along with various machinery equipment, prior to all the equipment been installed we tested 3 No spreader beams 1 No at 750 tons and 2 No’s at 700 tons.

Transporting some of the Modules from various locations in the Shipyard to site using the SPTM trailers, then lifted the Modules into place on the Jack-Up Barge, also off-loading equipment from barges and vessels again using the SPMT trailers.

I am also knowledgeable of Shell Lifting Codes and Procedures, Specifications and industry Practices. As well as the above I have also assisted Keppel to update their Scaffold Procedures to fall in line with the Shell Scaffold Procedures and British Standards, also with the Scaffold Inspection work. The Project was completed with 9.000.000 man hours without any lost time.

Project: AIOC/BP Chirag Oil Offshore Jacket Project

On my last project my duties entailed the day to day operations to supervisor all the Heavy Lifting /Rigging, Multi-Crane and SPMT Transportation and to make sure that the workforce of over 150 personnel are deployed to work and to ensure safe operations of all crane and rigging activities, responsible for all lifting on site relating to the Construction of a 18000 ton Offshore Jacket, with various lifts reigning from 300ton to the rolling up of the Centre Box side frame of 3250 tons along with the second side frame of 1850 tons also the Main side frames of over 2 x 6500 tons; this is incorporated by using a Strand-Jack system and the use of a 9 Crane Lift system the Lifting along with the placing of 2 x 850 ton Floatation Tanks on to Other duties also include in the construction of the Jacket are Multi Crane Lifts for the lifting and placing of launch runner and leg sections in a combine weight of over 700ton, along with the transportation,lifting,turning and positioning of 4 x 550 ton cluster pile guides in a Multi-Crane configuration also the lifting and placing of 2 x Tower Sections at 900 tons again each in a Multi Crane configuration, along the lifting and placing of various level section in the erection of the jacket, which are various weights, making sure that all rigging and other lifting equipment as well as all types of Cranes, Fork-Lifts and winches Certifications/records are kept up to date with the use of a Rigging Equipment Register and a 3rd party Inspection company and up to date color coding of all Lifting Equipment which is a requirement by Bos-Shelf /AIOC/BP Lifting Operation

Liaises with Project Construction Manager, Project Engineers, Project Procurement Manager / Coordinator, Project Safety Advisor, Project Quality Manager, Discipline Managers, Warehouse Supervisor, Sub-contractors, All Superintendents, Personnel, Fabrication Construction Engineers for the preparation of lifting/load out procedures, Maintenance Superintendent for good operational practice of equipment.

Procedures and Lifting Audits. Also working alongside the Engineering and HSE Department to Maintain HSE standards and QA/QC standards. During the Heavy Lifting Program, preparation of Lifting Plans/Studies, Method Statements and Work Procedures along with Heavy Lifting check Lists are completed Task Risk Assessments, TBT, RTC Pre-Lifting meetings prior to any lifting operations as required by Bos-Shelf and AIOC/BP standards. My last position was in a back-to-back secondment for Chevron in the management and interfacing in a JV KBR/Saipem along with the Heavy Lifting and Transport Contractors ALE and other local sub-contractors, in the offloading from the Ro-Ro and geared ships of heavy duty equipment and the installing of equipment on site, review all lifting and rigging and transportation procedures/drawings for Heavy Lifts up to 2240 tons using a Gantry Crane System and a Strand Jack System, also for Multi-Crane Lifts, and the Transportation of various size Modules from 600 ton up to 2800tons for jacking and packing also for skidding, review and document pre-lift briefings with members of the rigging crew, the operator. And other affected parties and ensure personnel are clear in their responsibilities and tasks during each operation. Inspect all lifting slings, rigging hardware, and rigging equipment in accordance with ASME standards for wear Identification and damage prior to their use on each lift, working to OSHA and KBR Lifting regulations. Inspection of ALE and all sub-contractors heavy duty cranes as well as there O/H/Cranes and Tower Cranes lifting and rigging equipment, test certification and crane operators, make sure that all the companies working on the site are in compliance with the governing documentation for all there cranes competency certificates, on-going Audit to ISO 9001 Participate, also take the lead in Technical Audits on all lifting equipment every 4 months on each contractor on site this was change later on in the Project to every 3 months as more local contractors arrived on site with their lifting equipment Review both the Non-Critical and the Critical Lift, Participate in all lifting and rigging Audits and crane related incidents and Accidents, also Root Cause Analysis investigations reviews, attend IIF Meetings as per Chevron/KBR procedures Having a working knowledge of various crane and lifting regulations both for onshore and offshore Rigging Package including the review of lifting and making out lifting plans equipment and cranes make sure that all SOP standards and procedure are in compliance, along with QA/QC work liaise and maintain good working relations with the Project Team, Freight Forwarder, Heavy Lift Contractor, Vendors and local Authorities to ensure planned and Timely delivery of materials to the Project Site. My duties as the Heavy Lifting Superintendent was to manage and interfacing with the Heavy Lifting and Transport Contractors Al-Jaber and ALE in the offloading from the barge and the installing of PAR and PAU’s on site, using both heavy duty cranes and SPTM’s for jacking modules in to place, this included reading the method statements of the Barge moorings, Transportation route approval of all sub-contractors lifting and rigging plans along with the TRA, review both the Non- Critical and the Critical Lift Rigging package, oversee and the coordination of the Transportation Contractor in support of the movement of Major equipment to site, hold and document all pre-lift meeting prior to any Lifting and Transportation operations taking place ensure compliance with site HSE Manager oversee the contractor’s operations throughout all lifting and transport operations, make sure all Rigging Equipment is Inspected, Including verifications of all rigging and lifting certification and color coded and is in accordance with ASME Standards.
