Provide acute and chronic medical care to inpatient middle adult and geriatric psychiatric patients. I performed medical H&P. The recommendations I provide in medical H&P are followed by the attending psychiatrist.

I manage acute medical issues that may arise during patients’ inpatient stay such as chest pain, sob, DVT/PE, electrolyte abnormalities, heart conditions, stroke, AKI, infections, falls, etc.

I ensure that patient’s chronic conditions such as HTN, HLD, DM, CAD, Stroke, other heart diseases, thyroid disorders, anemia and other blood disorders, kidney disease, are managed based on current guidelines and recommendations. I ensure that patients’ outpatient medications are reconciled during their inpatient stay and upon discharged.

I order and interpret initial an subsequent labs including CBC, CMP, TSH, LFT, HgbA1c, Lipids, UDS and EKG.

I order imaging such as XR, USN, CT, MRI if needed.

I order PT/OT/ST evaluation and treatment if needed.

Make consultations with other teams as needed

Nurse Practitioner Provider, Multiple Clinics and Infirmary of the Tucson Department of Corrections (DOC), Corizon Company

Provided primary, infirmary, urgent, and emergent care to male inmate populations.

I started as one of the providers rounding on about 30 infirmary patients three times a week for 6 months. The other 2 days in a week are clinic days in the different clinic yards (8 clinics) inside the correctional facility. Infirmary has about 60+ beds with patients that are of varying age range (early adult to geriatric), sicker, and has more healthcare needs. My duties in the infirmary involves interviewing patients, performing complete physical assessment or focused assessment, identifying healthcare needs, ordering and interpreting diagnostics, formulating differentials and diagnosis, planning and ordering medications or treatments, working with other healthcare team members, coordinating care, and making referrals if needed. I am also responsible on admitting patients from the different clinic yards of the DOC that must be monitored due to change or worsening of conditions, from other jail systems in Arizona, and patients returning from the hospitals, nursing homes or rehabilitation units. I am also responsible for discharging patients who have improved and no longer need infirmary care. My responsibility also involves responding to urgent and emergent needs of the inmates and making the decisions whether to transfer patient out of DOC to the hospital or not. We try to keep inmates that are sick in the infirmary for as long as we can effectively and safely care for them in the infirmary for economic and public safety reasons.

In March 2017, I was assigned a regular clinic with 800+ inmate population where I spend clinic time three days a week (Mon-Wed), one day in 2 smaller clinics—clinical detention unit with 200+ population and minor’s clinic with 200+ inmates that are from 14-17 age range (Thursdays), and every Friday rounding on infirmary patients. I see between 15-20 scheduled patients a day plus any patient that walks up to the clinic with request for diagnostic reports and with urgent/emergent needs.

As new inmates come in from outside DOC, I am responsible for doing physical intake assessment, identifying their health needs---acute and chronic conditions for follow-up, transferring inmates either to infirmary, behavioral unit, or the general population yard and assigning them medical scores so they could be properly housed.

I am also responsible in attending and stabilizing inmates with acute conditions requiring immediate interventions such as drug withdrawal, alcohol withdrawal, intractable seizures, cardiorespiratory compromise by prescribing medications for patient to safely go through withdrawal period, abort and prevent seizures, and by making decisions to either keep patient in the yard, or the infirmary, or to transfer them out to the hospital.

Between clinics and infirmary, I have also performed minor procedures such as incision and drainage of abscesses, wound debridement, wound care, removal of ingrown or infected nails, steroid injections, simple splinting, minor wound/laceration repair with suturing, and cryotherapy.

I also have on-call duty 24/7 for one week every 5-6 weeks for the entire Tucson Department of Corrections which housed more than 5000+ inmate population.

Staff RN/Relief Charge Nurse/Preceptor, Neurosciences Unit, University of Colorado Hospital
