Coordinate and provide direct care to patients with infusion/vascular needs in an Infusion facility, home, or alternate facility. Initiate Nursing plan of care treatment, evaluating patient progress towards goals. Performs direct care, patient treatments, infusions, administers medications, provides vascular access, dressing changes, and educates patients and families. Obtains and clarifies physician orders for plan of treatment, informs physicians promptly of any significant changes in patient’s condition, and provides written summary to physician within supplemental orders.
