The Transition Navigator operates under the supervision of the Manager Clinical Navigation and Engagement and works closely with the Patient Navigator (PNAV) and Client Solutions and Engagement (CS&E) teams to ensure that the Transition of Care (TOC) prescribed by the physician is followed throughout the patient’s episode of care. This position guides the post-acute care provider(s) through the patient’s episode of care by adhering to physician’s orders. This role focuses on performance improvement by monitoring clinical and financial outcomes and builds network relationships by data sharing and benchmarking. This role serves as point of contact for the patient care team to provide post-acute resources and assistance with accessing clinical and supportive care services within the physician’s prescribed course of care. Primary Responsibilities and Essential Duties:

• Communicate in a timely manner with post-acute care providers, physician practices, and the patient care team

• Manage routine processes and communications related to pre-admission and post-discharge care transitions

• Manage post-acute providers in accordance with the TOC plan

• Communicate with physician (or designee) to understand approved TOC

• Generate pre-approved physician orders from the TOC and delivers to acute and post-acute sites of care

• Use established clinical protocols as noted on the TOC for navigating patient’s site of care

• Enter provider communications, demographics, and other data into electronic databases

• Maintain regular communications at designated intervals with post-acute providers and patient care team

• Support health of patient throughout transitions in the continuum of care by open communications with the patient care team

• Ensure appropriate patient goals are established and monitored; care is received in the most appropriate setting; communication is shared effectively among providers; and standards for superior quality care outcomes are achieved
