I work in an immunogenetic lab for donor chimerism monitoring of post-transplant patients. By mastering the bench works, blood cell lineage isolation, flow cytometry, STR-PCR, fragment analysis, and semi-quantification of microsatellite amplification, I provided numerous chimerism results to help with post-transplant care for individual patients. I appreciate the extraordinary opportunity to strengthen my knowledge of transplant medicine and to extend my view of the essentials of clinical testing. The experience and specialized knowledge encouraged me to advance as an HLA lab professional through the American Society for Histocompatibility and Immunogenetics

Most Meaningful The first time I worked in a clinical lab, although I was experienced working in a research lab. The experience has opened a door for me to a new field or a new career. Graduate student

The non-surgical treatment of pediatric strabismus could save healthcare expenses and reduce surgical complications. We proposed that the exogenous growth factors could re-imbalance the agonist and antagonist extraocular muscles and align the visual axis. I injected the IGF1 or cardiotrophin-1 into the orbital cavities of baby chickens, measured the isometric strength in vivo for a week, and examined the morphologic alteration of treated muscles under a microscope. Under the guidance of dissertation advisors, I completed the study with publication on Invest Ophthalmol Vis Sci. 2010, May; 51(5): 2479-2486, and Pflug Arch Eur J Physiol. 2011, Mar; 461(3): 373-386. In addition to coursework, seminars, and journal clubs, I achieved a doctoral degree in Cellular and Molecular Pharmacology and Physiology in May 2011. I appreciate the opportunity of dissertation research, which supported my interest in clinical studies that aim to find evidence or therapeutic strategies for future medicine. Most Meaningful The first time I moved to the USA. I have experienced a typical process in which a new immigrant changes and learns to survive.

After graduation from medical school, I was a resident working in a university hospital. My duties covered inpatient care in the Neurology and Internal Medicine Department and the patients in the Emergency Room with suspected neurologic issues. Under the supervision of the attendings, I practiced medical history, performed a physical and neurological examination, ordered the lab and initial treatment, wrote the notes, and reported the cases. Also, I joined the epilepsy team, followed the cases with unique manifestations, and contributed to the related clinical research (Epilepsia. 2010, Sep; 51(9): 1669-1678). I appreciate the practice training opportunity in patient care, case discussion, journal club, educational conferences, clinical trials, clinical studies, and clinical/scientific writing (Journal of China Prescription Drug. 2005, Feb; 35(2): 47-49). The training strengthened my clinical knowledge and aroused my interest in clinical translational research.

Most Meaningful The first work experience after graduation from medical school. It was a real world where I practiced my clinical knowledge to serve patients and help teamwork. Summary writer

I joined the monthly COVID-19 virtual symposium and completed the summaries for more than two lectures. To understand the lectures in detail, I went through the major research articles of the research group to extract the group's main findings or focused areas. Each symposium usually had seven to eight lectures and invited nationwide or international researchers who conducted critical research studies on SARS-CoV2 with publications and many citations. The high-frequency topics covered vaccine-related immunity, the uncommon clinical manifestation of the critical cases, the antibody-targeted epitopes on the spike protein, the SARS-CoV2 genetic evolution, SARS-CoV2 clinical testing, neuropathologic findings of critical cases of SARS-CoV2, and so on. It was an excellent opportunity to touch with the newly developed technologies and the novel modification to the methods that led to the critical findings. The summaries I completed were posted on the university website in two weeks for archival purposes.

A one-month intensive refresher training covered the critical elements of the clinical practices in the U.S. medical system, such as healthcare communication, child abuse, medication-assisted treatment for Opioid Use Disorder, professional boundaries, cultural competency, high-value clinical care, and so on. Through the “hands-on” experience of simulated case studies and HPI notes, I have better understood the U.S. medical system and built communication skills with clinicians and patients. research volunteer

I worked in a nine-volunteer group to conduct in-person surveys of healthcare employees about COVID-19 antibody testing results, suspected symptoms, and PPE usage. Also, I joined the group writing task of evidence summaries based on the newly published scientific articles. During the early pandemic, the research teams in the Office of Public Health of NYC Health and Hospital designed the study to understand the SARS-CoV2 spread and provide evidence for the forthcoming public health intervention. It was a precious opportunity for me to understand the evidence-based healthcare system in the USA, apart from the clinical practice guideline. The study was published in BMJ Open 2021;11:e053158. doi:10.1136/bmjopen-202*-******. Li, Tian (12724608) MyERAS Application

A five-month clinical experience in a busy neurologic clinic provided more intensive training in neurology. I improved my skills in medical history, neurological examination, HPI notes, assisted neuralgia block injection, EMG test, nerve conduction study, and EEG test. The cases of neurologic diseases I encountered included neuralgia, epilepsy, multiple sclerosis, alcohol-abused brain disorder, dementia, and myelopathy. Interacting with neurologic patients strengthened my interpersonal communication with emotional patients or complicated cases and raised my responsibility and awareness of the value of healthcare under the US medical system. volunteer

I appreciated my social-work teachers' help with the replacement to study the multidimensional characteristics of dementia while practically focusing on group psychotherapy and the social and family impacts of dementia. To detect mild cognitive impairment and early-phase dementia, the Center for Cognitive Neurology conducted a series of clinical studies and community activities to educate the seniors and screen their cognitive functions. With those data and resources, the multidisciplinary professional group supported dementia patients and their families with therapeutic and psychosocial services. With that experience and literature review, I comprehensively understood that psychotherapy was one of the three major therapeutic strategies to improve the patient's life quality of chronic degenerative neurologic disorders. The genetic defects of voltage-gated sodium channels are associated with epilepsy. One of my postdoc research studies aimed to explore if the genetic variation in SCN1A gene 3’-UTR was associated with epilepsy. In this case-control study, we retrospectively recruited more than 100 epilepsy patients, analyzed their genetic sequences of SCN1A-3’UTR, and identified one of twelve genetic variants was significantly less frequent in male epileptic patients. The study was published in BMC Genetics (2016) 17:111. DOI 10.1186/s12863-016-0417-y. The other study of my fellowship was about gene expression of an accessory subunit of the sodium channel, SCN1B gene product, in in-vitro cellular models. Besides, as an investigator at a clinical site, I joined Phase III Perampanel (E2007) clinical trial in China for adjunctive therapy for partial seizures. I appreciate the valuable fellowship training for my critical thinking and problem-solving skills with increasing interest in clinical or translational study. Li, Tian (12724608) MyERAS Application
