Health Education Coordinator Senior

Supervise, train, evaluate and support 4-6 Risk Reduction Specialists embedded in harm reduction/needle exchange programs.

Liason with the community on Narcan Administration and training.

Crisis Management with the participants and families as well as other agencies for persons in crisis.

First contact for the Risk Reduction Specialists and will provide individual consultations to discuss difficult clients/cases, as needed.

Attend planning meetings and outcomes meetings on the local, state, federal level.

Assists in coordinating coverage of Health Education and Risk Reduction counseling in concert with syringe exchange program hours.

Specialists attend mandatory continuing education programs and trainings related to required interventions and strategies, and will conduct annual personnel evaluations for the Risk Reduction Specialists.

Provide program documentation on unique program services and reporting statistics and regularly evaluate case notes and data entry of Risk Reduction Specialists, providing additional training on quality as needed.

Assist the HRI Program Manager in preparing performance reports and narrative as needed, and will assist in data collection/entry into reporting database and attend trainings as necessary.

Provide Ryan White HIV/AIDS program services: Early Intervention, Outreach, and Health Education and Risk Reduction counseling for persons who are at high-risk for acquiring or transmitting the Human Immunodeficiency Virus (HIV).

Targeted HIV and Hepatitis C testing and counseling to persons presenting for services at the harm reduction programs to help the unaware learn of their status.

Refer persons found to be positive for HIV or Hepatitis C to medical care and treatment services at key points of entry for Outpatient/Ambulatory Health Services, Medical Case Management, Mental Health and Substance Abuse Services.

Counsel persons at high-risk for contracting or transmitting HIV on how to reduce the risk of HIV transmission and will work with individuals to assess individual risk needs and connecting them to appropriate resources as needed.

Provide information about medical and psychosocial support services and counseling to improve the participants health status.

Distribute patient education materials (pamphlets and books) as well as risk reduction supplies (sharps containers and condoms) to persons at high risk for contracting or transmitting HIV.

Oversee the daily functioning of the foster care, independent living, and behavioral health programs. Crisis intervention for foster parents and youth in the program. Provided therapy services using Trauma informed care as well as CBT on a regular basis. I also began a Behavioral Health Program in 2016 in our office.
