Skilled Nursing Flow Sheet – Clock In / Clock Out

This guide will provide information about Clocking In and Out via a Skilled Nursing Flowsheet using the HOPE device..

To Clock In

Select the Patient.

If you do not see the patient in the dropdown contact your branch office. They must assign you to the patient in myUnity before you will be able to document on the patient.

Select the Skilled Nursing Flow Sheet form.

A Skilled Nursing Flow Sheet Quick Start screen will pop up.

oVerify the Visit Start Time is your actual Clock In time. Edit if necessary.

oTap the Clock In & Create button

The first time you use the ‘Clock In & Create’ button you may be presented with a popup asking to use your location.

You must select Allow.

oAveanna must comply with new regulations obtaining GPS coordinates for all Clock Ins and Clock Outs.

This is a one-time selection.

If you do not see this popup, please ignore.

Once you have successfully opened a Skilled Nursing Flowsheet, the Clock In time that appeared in the Quick Start screen will populate the Time In on page 1 of your documentation:

To Clock Out

To clock out at shift end, use the Clock out button.

**Note: You will not be able to select Send to Office until you have Clocked out.

Verify the Time Out is correct. Edit if necessary. Select Clock Out

The time selected in the Clock out screen will populate as your Time Out on page 7.

NOTE TO OFFICE PERSONNEL:

The nurse is still able to edit date and times on the skilled nursing flowsheet after using Clock In or Clock Out. The fields are not locked down.

Time In and Time Out = EVV Times

EVV/API calculates the thru date on night shifts so start and end date will continue to be accurate on overnight shifts.
