OBJECTIVES

Prescriptions audit both for inpatient and outpatient department.
Decreased hospital readmission rates and increased patient medication compliance by performing hospital medication reconciliation review.
Ensured appropriate drug and dosage are used according to the diagnosis and protocol.
Screening and checking prescription.
Provided the clinical advices to fellow health professionals.
Provided ward based services to the medical wards.
Assisted primary care teams through effective communication patient and other care team members to get patients to clinically significant goals.
Assessed desired outcome of chosen therapy and intervene when appropriate.
Recommended alternative therapies when drug- drug or drug disease interaction existed.
The adverse drug reactions were observed reported analyzed using WHO and Naranjo’s scale and communicated with CDSCO.
