In identifying barriers to health care for the among African refugees’ population to Providers, educators or interdisciplinary team.

Utilize social service skill in helping triaging the client needs with care coordination. Support of these services includes screening to identify patient needs, assisting with connections to community resources and help with ongoing care plan development and follow through as necessary.

The department was dedicated to promoting vibrant, healthy and safe communities through Environmental Prevention strategies. Environmental Prevention plans addressing community norms and policies related to alcohol and other drug (AOD) problems, in collaboration with other project staff, community groups, community leaders, prevention agencies, prevention provider workgroups, law enforcement and policy makers.

My ESSENTIAL FUNCTIONS was : 1. Help develop and implement a strategic Environmental Prevention plan to further substance abuse prevention goals;

2. Collaborate, create, and cultivate partnerships among community groups to help implement ATOD prevention campaigns;

3. Conduct presentations on alcohol and other drug prevention issues to community groups;

4. Participate in data gathering activities; to include risk assessments, focus groups and community surveys;

5. Participate in selected collaborative and other community meetings on a regular basis;

6. Provide training and technical assistance to community partners to support prevention campaigns;

7. Identify community leaders and solicit their participation as spokespersons for media advocacy-related activities;

8. Establish partnerships with law enforcement agencies, secure help in gathering enforcement data, in conducting alcohol outlet compliance checks and in advancing policy goals;
