• Experience with seniors, experience with assessment, diagnosis, and treatment of chronic illnesses.
• Performs routine and emergency assessments related to outpatient palliative care visits.
• Collaborates/Communicates with Medical Director, Attending Physician, DCS and IDT.
• Educates patient and caregiver regarding:
o Care of patient.
o Disease process.
o Dying Process
o Symptom control.
o Wound care.
• Perform pertinent history and physical examinations of patients to assess their physical, emotional, and/or social needs.
• Establish medical diagnosis and determine a plan of care for short term health problems and/or chronic stable health problems.
• Obtain, perform and examine laboratory tests including, but not limited to bacteriological smears, KOH and saline preparations, pregnancy test, strep and urine examinations. Obtain, perform, and interpret routine laboratory studies such as hematocrit and urinalysis.
• Order and interpret routine diagnostic studies including, but not limited to blood chemistries, culture and sensitivities, radiological examinations, non-invasive cardiovascular studies, neurological screening and pulmonary function tests.
• Order necessary medications using clinical judgment and the specified references.
• Perform therapeutic, corrective procedures including, but not limited to, the removal of skin lesions, incision and drainage of superficial abscesses, debride, clean, suture, and treat superficial wounds, and apply splints as needed.
• Counsel and educate patients in heath maintenance, diet, exercise and management and prevention of illness.
• Refer patients to appropriate licensed physicians, clinics, or allied health care providers for the purpose of chronic management or consultation.
• Act in emergency situations and provider basic cardiac life support until the physician or other emergency personnel are available.
• Do Home Visits and Skilled Nursing Facilities (SNFs).
• Performs other duties as assigned.
