As the Oncology Navigator my role is to identify potential barriers to care that patients may experience. Some of the barriers are: patients and families psychological stressors, diverse cultural values, transportation, child care, financial problems, lack of insurance coverage, and end of life issues. I will navigate the heath care system with the patients and become their advocate. I work with patients and families who have a new cancer diagnosis or are diagnosed with recurrent cancer. I provide emotional support and resources to patients and family members. Provide short term individual, family, and group counseling to the oncology patients. Provide brief individual counseling to a variety of individuals from various cultures, ethnic, and diversified backgrounds. I assess the psychosocial functioning and needs of patients and their family members. I speak with family members about end of life issues and educated family members on Hospice care. Provide crisis counseling when the patient or family member has a difficult time coping. I provide in services to new nurses about my role as the oncology navigator.

Provided assessment, management, and advocacy services to homeless families with children. Worked closely with professionals from numerous community organizations, with volunteers from religious congregations, as well as staff members. Provided counseling to at risk youth and children who were in the program as well as family counseling. For example, on many occasions I accompanied one of the adolescents to a court hearing and advocated on his behalf in the school system. Conducted weekly case management meetings with each family to review case plan, tracked progress, set goals, dealt with a variety of issues, provided guidance, family counseling, and a variety of community referrals. Helped the families through self – sufficiency by helping them find resources and move towards independent living. Helped the families with their budget, finances, parenting, stress relief, and life skills. Managed the compliance of the families using the program guidelines and reported violations. Exited families from the program and followed appropriate paperwork. Implemented policy regarding the mentoring program including screening, training, oversight of mentors and matching appropriate mentor with families. Managed and screened all prospective families by developing a needs assessment, case plan, using telephone and in-person interviews, and conducting orientation into the program. Provided crisis counseling to families as well as volunteers. Trained volunteers, mentors, and supervised their interactions with families who were in the program. Provided many in services to the community regarding the program. Supervised social work interns, mentors, congregation coordinators, and volunteers on an on-going basis. Provided monthly reports to board members as well as coordinators. Built relationships with supporting and referring agencies, helped with fundraising events. Was trained in Sales Force for record keeping.

Worked with patients who were diagnosed with a variety of psychiatric diagnosis, chronic disabilities, cancer, and other health problems. Worked with patients who had debilitating illness and a combination of medical and psychological diagnosis. Dealt with issues such as grief and loss, depression, anxiety, suicidal ideations, drug and alcohol dependency, alcohol abuse, and marital conflicts. Many of the Hospitalist and Oncologist would ask me to provide a psychological assessment on the patients who were not coping well. Continued by providing emotional support and resources. I spent most of my time on the oncology floor and provided individual, family, and group counseling to the oncology patients. Provided therapy to children whose parents had a diagnosis of cancer. Worked with patients and families who were experiencing a variety of psychiatric, medical, and social problems. Have worked with patients who had an acute, chronic, and traumatic illness. I provided individual therapy to a variety of individuals from various cultures, ethnic, and diversified backgrounds. Assessed the psychosocial functioning and needs of patients and their family members. I am able to formulate and implement treatment plans. Dealt with issues such as grief and loss, depression, anxiety, suicidal ideations, drug and alcohol dependency, alcohol abuse, and marital conflicts. Spoke with families about end of life issues and educated families on Hospice care. Helped the Case Managers with resources for cancer patients as well as patients who were homeless. I am also knowledgeable about the medical and mental health diagnoses of the patients and common medications. On many occasions, I provided crisis counseling to patients as well as staff. Assisted in helping patients create advance directives and work closely with chaplains by interceding with spiritual care. Supervised mental health students from local university. Worked closely with oncology volunteers and supervise them as well. Have the ability to communicate effectively both orally and in writing with individuals for a variety of backgrounds. Worked closely with labor and delivery unit and provide grief counseling and well as resources.

September,2005- August, 2007 SHARE Horizons Pregnancy and Infant Loss Program Hospice of Palm Beach County.

Worked with families who experienced the loss of a baby through miscarriage, stillbirth, neonatal complications, and trauma, under the age of five. On many occasion, I received referrals from the Department of Children, when a child and family suffered a traumatic event, such as drowning. Provided individual, family, couple, and group therapy. Worked with grieving children and adolescents by providing family and individual therapy in the home and school setting. Assisted in translating a variety of educational material into Spanish. Conducted in-services to local hospitals on the SHARE program. Provided families with information and education. Assisted in coordinating the Angel Candlelight ceremony for parents who experienced the loss of a child. Attended training for Crisis Response and worked as part of the crisis intervention team. In order for my patients to start creating memories, I would go to the morgue and make hand and feet molds of their diseased child for them to keep.
