Health Homes Care Manager

Responsible for the assessments, reassessments,coordination's of care and

services. Includes ongoing monitoring of all appropriate and effective person

Centered care plan, member education and care management. Regularly

communicates with the members PCP and other providers and integrates the

member,care givers and other provider feedback into the assessment and

planning. Face to face contact.Conducts home visits,site visits to shelters. Also,

visit residential and outpatient programs to outreach and enroll patients. Engage

and support patients in community treatment. Responsible for referring patients

to appropriate agencies, with specific focus on referrals to specialty care and

public health insurance programs, as needed and appropriate . Consult with

community partners to link patients to services needed, including translation,

transportation assistance. This may include participation in case conferencing.

Provide services to help make sure that everyone involved in an

individuals medical care, is working well together and sharing information that

supports the patient’s needs. Responsible to help coordinate not just medical,but

mental health, substance abuse services,housing and social services needs for

the individual. Maintain a case load of 50 to 55 patients.

5-07 to 2012 HOPE CENTER (COBRA, Case Manager) HIV population

I worked with the HIV/AIDS population. Responsible for the intake,

assessment of needs, service planning, service plan implementation, service

coordination, monitoring and follow-up, reassessment, case conferencing, crisis

intervention, harm reduction, CBT, MI and case closure. In addition to assisting

clients to access and maintain specific services, the case management activities

may include negotiation and advocacy for services, consultation with providers,

navigation through the service system, psycho- social support, supportive

counseling and general client education. The goal of case management is to

promote and support independence and self-sufficiency. As such, the case

management process requires the consent and active participation of the client

decision making, and supports a client’s right to privacy, confidentiality, self-

determination, dignity and respect, nondiscrimination, compassionate non-

judgmental care, a culturally competent provider, and quality case management

services. Also, assist the client to maintain virology suppression.

6/2004 Yonkers Residential Center (YRC)

Present Counselor/Child Care/Driver

Responsible for the supervision of (16) adolescents that are mandated by the

courts, for drug abuse and trouble with the law. Maintaining the structure of the

program that supports the goals of each client; interact with the clients in all

areas of their treatment. Also, facilitated groups and kept an hourly log and head

count. Take clients on field trips and cloth shopping. Supervised in house visits by

clients' families. Prepare incident reports, infractions, take random drug test and

report them to program Director. Conduct room checks to ensure that no drug

paraphernalia is brought into the program.
