Report to Alfred Schuls, Director, Cardiovascular Services

222 South Herlong Avenue

Rock Hill, South Carolina 29732

January 2012 - Present

Current manager of three Cardiac Cath Labs and an eleven bay holding area with a staff of ten nurses, six technologists and one technician. Provided staffing services to the operating room for AAA repair. Reviewed by Joint Commission in September 2012 and Piedmont Medic Center was named one of the nations top Performers on Key Quality Measures. In the ten months of 2012 we have provided cardiac, EP, and vascular service to 2,589 patients and billed for 4,867 procedures. Currently using the Meditech system we will transition to the Cerner electronic medical record by June 18, 2013. The three Cardiac Cath Labs are GE X-ray with MacLab for physiological monitoring. Door to balloon time average is 65 minutes using the NCDR CathPCI Registry to track data quality metrics. This is a stable environment with growth from a spill over from Charlotte Metropolitan area and a competitive friction from North Carolina Hospitals and physician groups. Developed new Electro Physiology Lab to remain competitive. March 2013 opening. Tenet Hospital System with Med Assets for material management, which provides unique resources use Opticell. May 2013 changed from Meditech to Cerner EMR.

Invasive Labs Manager

Report to Katherine Whelan, VP

1500 South Main Street Fort Worth, Texas

February 2011- October 2011

Managed the Cardiac Cath Lab, Special Procedures at the County Hospital of Tarrant County, JPS Health Network. I was recruited to develop and expand the Invasive services at JPS. The short- term goal was the construction of a larger recovery room where the old ED had existed. The long-term goals were the training and development of the support staff to meet this increased demand and services and build a new hybrid lab closer to the Emergency Department. Programs to provide ICD and expand special procedures were anticipated. These goals and programs were in process when I was laid off due to institutional financial short falls.

The Studer Group was the quality Assurance program at JPS. JPS Health Network is a county hospital for Tarrant County, Texas. The emphasis was quality service and satisfaction for patients and physicians, while holding costs and FTE positions to a minimum, and I achieved this by providing three-man call coverage in the lab in place of the four- man coverage that was being utilized.

I directed a growing Cardiovascular and Specials Procedures labs, which had one cardiac cath lab and two angio suites. There was a staff of eight in the cardiac cath lab and nine in the special procedures area. Services include Cardiac diagnostic and interventional (i.e., PTCA, STENT, Rotorblator, Athrectomy, and IVUS) cardiac catheterization. Vascular diagnostic and interventional procedures were provided at JPS, including of peripheral, carotid and cerebral vascular circulatory systems which were performed by interventional radiologists and vascular surgeons.

Emergency heart and brain attack services were available 24 hours by seven days. JPS Vascular Surgeons provide a majority of the carotid stents in the Fort Worth area. Drains were inserted in Specials with fluro, CT, and echo. Permanent pacemakers are implanted in the cardiac cath lab. TEE's and cardioversion are also provided in the Cardiac Cath Lab. The X-ray Systems were Siemens. A Sensis Siemens physiological recorder was used. Kronos was employed for timekeeping. Inventory was monitored by SpaceTRAX system. Lawson was the ordering system. This was an amicable separation.

Cardiovascular Technologist

Report to Katherine (Kitty) Zell

200 Lothrop Street Pittsburgh, PA

November 2003-February 2011

Provided monitoring and circulating duties in a unit that has five cath rooms and performs more than five thousand procedures a year. There are six technologists, eight nurses, two hem techs, and fourteen physicians to provide services at this teaching hospital facility. Cardiology fellows scrub in with the physicians. Diagnostic cases include endomyocardial biopsies for heart transplant patients, pressure and flow wires to evaluate lesions. Invasive procedures include drug eluding stents, Rotorblator, Angiojet, and filter wires. Most electro physiology studies are done by the EP Labs with their techs and nurses but we are called in occasionally for temporary and permanent pacemakers. PFO and ASD closures are also provided in our labs. Intra Aortic Balloon Pump and Angiojet devices are placed to assist cardiac output as well.

During my tenure I have helped develop policies and procedure. I have authored two articles for Cath Lab digest, developed spreadsheets for scheduling, investigated and made recommendations for reducing costs in the use of biohazard disposal (red bag) of cath lab waste. We are no longer using the sharps containers for the disposal of glass containers for contrast and nitro. This is a change of twenty- two cents a pound for disposal to just two cents a pound. I am a contributing member of the cath lab in many areas.

Paralegal Report to John Burt, Esquire

Feb. 1997-Nov 1999

Directed a growing cardiovascular lab, which provided 1,274 patient procedures per year. Services include Cardiac diagnostic and interventional (i.e., PTCA, STENT, Rotorblator, Athrectomy, TEC, and IVUS) cardiac catheterization. Vascular diagnostic and interventional procedures in the peripheral and cerebral vascular circulatory system were provided. Emergency heart and brain attack services were available 24 hours by seven days. Aiken has one of the few labs providing carotid stenting. Permanent pacemakers are implanted in the cardiac cath lab. Renovated the cardiac cath lab in 60 days. A modular cardiac cath lab was leased to keep services uninterrupted. The GE X-ray System was replaced with a TREX and digital X-ray system. Q-cath physiological recorder and Summit Database are currently used. The lab was Y2K compliant.

I was recruited to develop and expand the cardiac services at ARMC. The short-term goals were the construction of two new cath labs and recovery room. The long-term goals were the training and development of the support staff to meet this increased demand and services. Programs to provide Electro physiology and AICD were anticipated. These goals and programs were in process when I left for Pennsylvania. Service Excellence was the quality Assurance program at ARMC. United Health Systems is the corporate owner of this for profit hospital. The emphasis from the corporate office was quality service for patients and physicians, while holding costs and FTE positions to a minimum, and I achieved this by providing coverage in the lab and on call in addition to performing my administrative duties.

Director of Cardiac Catheterization Laboratory and

Director of Cardiac Rehabilitation Service

Report to Joan Gussic, V. P. and William Casey, CEO

Conemaugh Hospital

1086 Franklin Street

Johnstown, PA 15905

June 1990-Nov 1992

Developed two new services for a 412 bed regional teaching institution. Planned and directed the development from blue prints to operation of the Cardiac Catheterization Laboratory. Wrote requests for proposal, reviewed requests, and negotiated contracts for equipment and supplies. Recruited, hired, and trained staff for a Cardiac Cath Lab. Directed the development of policies, procedures, and quality assurance for both services. Wrote the business plan for Cardiac Rehabilitation Services with proforma and cash flow forecasts. Utilized PERT and GANT charting for the planning of Cardiac Rehab and Cardiac Cath Lab programs.

The development of the Cardiac Cath Lab facility required $3.1 million and came in under budget. Catheterization started on November 15, 1990, within two weeks of schedule. The open-heart surgery program started the next week. The revenue budget was 257% above prediction at six months, the result of 647 cardiac caths and 107 angioplasties (PTCAs). This bountiful response let me work clinically as well as administratively. To meet the record demand a second Cardiac-Cath Lab was developed and staffing increased at a cost of $1.6 million. On January 25th, 1992, a Siemens Hi-Cor was operational. To date more than 3,063 diagnostic caths have been performed and 570 PTCAs, as well as seven Athrectomy procedures.

Conemaugh's Cardiac Program enjoys a reputation of high tech equipment in providing full service for both diagnostic and therapeutic procedures. The greatest challenge has been with staff development and consumer confidence in the new programs. There is an on going emphasis on cost containment. Reduced inventory with just-in-time organization has been our greatest ally. Our Cardiac Service has been highly successful for all associated with the new programs.
