County. Chaplain of Facility Team assessed Patients and Families in
Hospitals, nursing homes, assisted livings, boarding cares etc. their spiritual
Needs and end-of life issues. And followed up according to Interdisciplinary
Team plans of care including bereavement up to 6 months. Provided spiritual
Support to team members.
