As a requirement of the Master’s in OT program at Chicago State University, undertook three levels of fieldwork assignments with typical populations to be likely encountered in professional engagements. All positions are under the immediate guidance of clinical instructors.

PRE-LEVEL I September 2013 – April 2014 Engaged in supportive initiatives as an introduction to likely working environments as an OT.

Levy Senior Center Developed a free computer education class with a cohort of students for this public center for the elderly and retired in the City of Chicago.

Trinity UCC Daycare Designed and implemented a short learning course geared toward future planning with a cohort of students for typical developing children.

Midwest Brain Injury Clubhouse Assisted staff with daily programs and activities for small groups of individuals who have experienced traumatic brain injury (TBI). Also developed a treatment plan for one individual to assist in money management and shopping strategies.

LEVEL I September 2014 – April 2015 Observed, shadowed and assisted working therapist with daily clientele duties.

Step By Step Care Group Assisted OT with home health early intervention clients ages up to 3 years. Performed childhood evaluations, developed client centered interventions, and documented daily treatment sessions. Typical caseload diagnoses: developmental delay and Down syndrome.

Genesis Rehab (North Chicago) Assisted OT with home health and outpatient OT services primarily tailored towards the elderly. Duties included assisting with new patient evaluations, performing patient assessments/re-evaluations, developing and carrying out daily treatment interventions, and helping to provide documentation for the clinical instructor. Typical caseload diagnoses: stroke, orthopedic injuries, and Parkinson’s disease.

LEVEL II October 2015 – March 2016 Acted as working Occupational Therapist under supervision of Licensed Occupational Therapist by evaluating clients, developing treatment plans, and providing occupational therapy interventions for the site’s clientele.

Human Resources Development Institute Inc., Williams Consent ACT Team Worked as an independent Occupational Therapist with minimal supervision of licensed therapist for six clients with cognitive disabilities to reintegrate into independent community living. Also provided group sessions for all 55 clients served by the Williams Consent ACT Team as well as staff in-services. Typical caseload diagnoses: schizophrenia/schizophreniform, substance abuse, and general anxiety disorders.

John H. Stroger, Jr. Hospital of Cook County (Cook County Hospital)- Worked as independent Occupational Therapist with supervision of licensed therapist for inpatient, acute care OT services primarily. Duties included performing new patient evaluations, patient assessments/re-evaluations, developing and carrying out daily treatment interventions, and helping to provide discharge recommendations. Trained and competent in PowerChart electronic documentation system. Caseload varied widely with different diagnosises but frequently encountered: 2nd/3rd degree burns (ICU), gun shot wounds, MVA trauma/fractures, lower extremity amputation, Stevie Johnson Syndrome, pneumonia, CVA, and spinal stenosis.
