Personal statement

The skilled master of conquering pain is an anaesthetist. It’s a speciality which deals with quick thinking and multitasking. It makes us be always vigilant thinking of possibilities of what might be the cause for the change in the vital parameters. These five years of my anaesthesia experience has always engrossed me and taught me this each and every day.

The speciality offered the opportunity to combine and bridge many aspects from medicine to emergency care. I pursued my career in anaesthesia by finishing my Masters in Anaesthesia and stood second in Annamalai University.

The specialty offer a different type of doctor patient relationship where the rapport has to be created within a short period and the one thing that runs through the patient mind is pain. The ability to adapt both with the patient as well as the operating surgeon is one of the qualities that I learnt the most in these years and it has helped me to move forward in a productive way. The field made me develop the skills of making decisions and change the treatment depending on the current status of the patient. As a critical care provider the basic human physiological aspects made me learn what drug to give to make the patient improve and increase his survival aspects.

I had various opportunities during which I had experience in intercostal nerve block for flail chest injury, blind awake intubation using laryngeal blocks for mandibular fractures and anticipated difficult intubation. I have done regional blocks for upper limb trauma (interscalene, supraclavicular axillary) for pain relief before shifting to operating room. I have done retrograde intubation using epidural needle and catheter technique in a patient with micrognathia and restricted mouth opening patient who needed to maintain airway. I have tried cervical epidural for thyroid surgeries with using 1% lignocaine with the supervision of my senior professors and tumescent anaesthesia for large fibro adenoma without distorting the breast lump. We have conducted various CMEs and workshops.

The cardiac anesthesia team gave me a totally different exciting experience of working in CABG surgeries and congenital heart disease cases. My intensive critical care rotation allowed me to learn about ventilators and the dynamics of the cardiopulmonary system that was necessary in long term intensive care patients. It is here that I learnt the various facets of combining skills with the academic knowledge in managing critically ill patients with invasive monitoring and managing emergencies.

During my neuroanesthesia I have even tried comparing entropy and BIS for patient recall, emergency manual in line intubation for a patient with head injury. I have done cases like cerebral aneurysm keeping an eye on vitals during the temporary and permanent clipping of the aneurysm. I have done cases supervised by my professors for cerebral tumours like tubercular sella menigioma,moya moya disease, schawanomma and craniopharyngioma. The most interesting case was a patient brain stem tumour where the resection was done under so much precision as the surgeons were operating near the cardiac and respiratory center. I was also posted in separate electroconvulsive and radioimaging sedation postings with propofol, midazolam and dexmedetomidine as continuous infusion.

The field made me groom both as a team player and a team leader making me flexible to work as a group. I love to teach, learn and was even selected as an instructor for BLS/ACLS organized by AHA in India. I stood second in our university held exam for Masters in Anesthesiology April 2013.

I have finished my introductory course in pain management and have done cases like facet, transforaminal, cervical and caudal epidural, sacroiliac joint blocks using C-arm for chronic pain management.

I would always like to learn the various aspects in giving the patient a better care which needs continuous learning. I would always want to join a team of health professionals who aim at making a difference in the society.
