Self Employed 1983- 1992 East Wake Eye Clinic Zebulon,NC I practiced Full Scope Optomery and treated every imaginable eye entity that you can treat in NC. - I sold the office

to Dr Bolick and it is now part of Eye Care Associates

Raleigh Eye Center 3320 Executive Dr Suite 101-****-**** I practiced full scope optometry

and the contact lens specialist there. There was a fair amount of co-managed cataract and

LASIK surgery. I was paid on production and was responsible for my own lane of

equipment.

Contact Lens Institute 2012 Dr Hayes wanted me to come in and start taking over his patient

load and to bring new patients with me from the previous practice. The details of the setup

seemed to not fall into place and we parted ways amicably after 7 months.

Fox Eyecare - Dr William Fox I did fill in work and worked at 4 different locations of Fox

Eyecare in Raleigh, Greensboro and Goldsboro.

Ocular Management Services - Owned by an Asheville,NC Ophthalmologist we provide the

onsite independent optometry services for National Vision in the America's Best Contacts

and Glasses. I opened up the Durham location and got it started for them. Now I am full

time at the Pleasant Valley Promenade location. It's a very busy place and a fast pace to

optometry care for their patients. I started work August of 2013 and currently work 5 days

per week though at times it has been 6 days per week. Their bonus system has not lived up

to my expectations.

I am a warm and personable practitioner and I like taking excellent care of all my patients. I have a good work ethic and get along with staff members extremely well. I like teaching staff

about all aspects of great patient care. My personal hobbies include avid golfer, snow skier,

traveling, cooking, and home remodeling. I have a house I am working on right now walking distance from North Hills Shopping Center inside the Raleigh Beltline. I have been in Raleigh

for 30 years and I know a little bit of everyone around town.
