Staff Pharmacist (Main Pharmacy, September 2013- September 2014)

-Fills prescription and dispenses medicines

-Prepares and compounds sterile preparations such as intravenous medication admixture, hydration/ electrolyte therapy and total parenteral nutrition for patient

-Provides drug information to medical and paramedical staff

-Dispenses dangerous or prohibited drugs to in or out patients with corresponding requirements and records it in the Dangerous Drug Book.

-Prepare request for price changes, new codes of new item/medicines and records adjustment made

-Receives credit memo or returned medicines and evaluate in accordance to the returned policy.

-Provides drug information and answers inquiries from doctors, nurses and allied services

-Monitors and record all Referrals of Emergency Purchases insisted by Prescribing Physicians.

-Monitors Restricted Drugs ordered by physicians and approved by Infectious Disease Consultants.

-Checking of expiry dates and labels of the stock medicines

ER Pharmacist (October 2014 – present)

-Fills prescription and dispenses medicines to outpatient

-Counsels patients upon discharge on their home medications to ensure better understanding of their prescribed dosage regimen

-Provides drug information to medical and paramedical staff, patients and their relatives or caregivers

-Trains new pharmacy personnel in the policy and procedures of the hospital
