Aurora Behavioral Health (Behavioral Health Tech – BHT)

My duties at Aurora are that of a Behavioral Health Technician. I assist and monitor patients who are suffering from a variety of mental illnesses such as schizophrenia, bipolar, personality disorders, alcohol and substance abuse – detoxing, PTSD and adolescent population. I observe and monitor every 15 minutes the location of the patient, for patient safety. Assist and monitor patients to and from group sessions and visitation with family and friends. Take daily vitals on the patients, record and submit any abnormal findings to the nurses on the unit. Prepare all paperwork for the next oncoming day/shifts. Assist with basic hygiene and ADL’s. Continuous monitoring, supervision of patients, and recording of such, every 15 minutes. Perform 1:1 suicidal watch and assist with any seclusion and restraints, when deemed necessary. Train new BHT’s in completing their shadowing the first couple weeks. I will elaborate further, upon request.

Valley Hospital, 3550 East Pinchot, Phoenix, AZ (Mental Health Tech – MHT)

Dates: March 10, 2013 to March, 2014

Primary duties as a MHT are that of assisting with patients being admitted into the hospital who are suffering from depression, personality disorders, schizophrenia, bipolar disorders, detoxing, chemical dependency, etc. We observe and document any changes in the patient’s appearance, behavior or ability to work toward their specific treatment plan goals. Communicate changes and patient’s issues to the nurses. We supervise and assist patients in all areas such as personal hygiene, patient safety, transferring patients to and from all groups, to all of their meals in cafeteria. We check patients for contraband and manage the patient’s valuables and belongings. Complete all of the vital signs twice a day, and give to Nurses to administer their medications. Act as the patient’s advocate and assure that patient’s rights are upheld. Comply with the Employee Code of Ethics; adhere to all facility personnel policies and procedures. We continuously ensure patient safety through continuous checks and supervision of the patients. We, as Mental Health Techs work closely with other Techs, Psychiatrists, Medical Doctors, Social Workers and Nurses.

Dates: April 21, 2008 to April 18, 2011 - I resigned to relocate to Arizona w/family

I served as the Psycho-Social Rehab (PSR) Department’s Coordinator. I was responsible for a multiple of duties in that department. I hired, trained and outlined duties for my assistant in that PSR program. I was ultimately responsible for coordinating, directing and carrying out all of the daily operations of the PSR Program. Supervised between 10 to 35 patients coming in on a daily basis. We overlooked and assisted patients suffering from Bipolar, Schizophrenia, Personality Disorders, Depression, etc. in accomplishing different curriculums which were outlined and designed to help them with their personal hygiene, fire safety in their homes, patient safety, preparing meal lists and grocery shopping, applying for employment, helping them develop a resume for themselves and so on. I was responsible for developing and outlining each week, different curriculums for these patients. I taught some of these

classes myself. Was responsible for all daily, monthly and semi-annual reports to administration. I was involved in clinical meetings twice a week with Psychiatrist, Psychologist, Nurses, Social Workers and Counselors to give updated reports on the direction and progress of the PSR Program and the patients attending. Traveled and attended all pertinent workshops, seminars and conferences related to mental health.

Connections, Inc. (RSVP/SCP Director)

Dates: May, 1995 to April 15, 2008

My title was Director of the Retired and Senior Volunteer Program. I was responsible for all daily duties and operations of this program. I supervised four employees that were assistants and clerical support for the programs. I hired and trained each of these individuals as well. I also was in charge of total recruitment for the programs in hiring Senior Companions for individuals who were home bound, disabled and needing daily assistance with their ADL’s. Completed full assessments on persons suffering from all types of mental illnesses and placed volunteers in group homes, hospital sites and at independent living homes. Persons that I worked directly with were persons suffering from Schizophrenia, BiPolar, Personality Disorders, Eating Disorders, and suffering from all types of depression and PTSD. I screened, hired and trained all volunteers in working with this population suffering from mental illnesses. I was responsible for developing and carrying out all of the training packets, did outreach in the community, recruiting persons 55 and over and also responsible for developing worksites for these individuals, trained them and made all of their placements out in the community. My work was set out for all non-profit, charitable and civic organizations. I worked closely with the programs Board of Directors and reported to them on a monthly basis. Developed all of the budgets each year for each of the programs, as well as completed all of the grant writing. I was accountable for all of the monthly, semi-annual and annual reports to the State Agency of Aging, as well as to the Corporation for National Service, which were located in Santa Fe, New Mexico. Traveled extensively to all In-State, Out-Of-State, Regional and National Conferences which our programs were all affiliated with. I supervised 15 Senior Companions, 48 homes where they were placed and also had 240 volunteers on the Retired Senior Volunteer Program, which served some of the 85 non-profit organizations that were registered with our programs.

Received an AA Degree in Human Services, also received several other certificates specializing in Alcohol and Substance Abuse classes, Counseling and Mental Health. Completed the CNA classes and am current with my CPR/Basic First Aid Certificate, and CPI. Took one year of Nursing studies and currently am planning to return to Nursing school to finish the RN program.

SKILLS and STRENGTHS:

Speed Typist – type around 90 WPM, exceptional organizational skills, people and computer friendly. Computer programs include WP, Excel, Data Input, Tracking Programs, Internet, Spread Sheets and Surgery Scheduling Experience. I have proven to be a self-starter, a quick learner, am adaptable and welcome to change. Am open to any travel and furthering my education, if needed.
