Patient Diagnosed: Dementia, Asthma, and Prostate cancer

• Assist patient with his morning care and daily routine

• Check for his IV catheter, IV solution and IV food

• Check for any embolism of his hands and arms at the site of the IV catheter

• Check for his normal breathing and encourage for breathing exercise

• Assist patient for his daily exercise (range of motion)

• Compliance for his medication but before taking his medication, I checked first his vital signs to

make sure for his good health.

• Clean up spills on the floor in order to prevent accidents; participate in safety drills

• Always check and clean for his suprapubic catheter to avoid any infections

• Keep linen and utility area clean and in order, empty and clean urinals and bedpans, empty dirty

linen.

• Record data on his weight, urine and bowel movement.

• Observe signs and symptoms of hypoglycemia of my patient

• Encourage fluid intake and monitoring urinary output.

• Encourage patient to eat full meals and snacks and check for his IV food.

• I always checked his skin especially his feet to make sure no presence of wounds and advise him

to wear socks to cover his feet for his safety.

• Help patient to elevate his legs if he feel pain to his knees and assess his joint mobility, range of

motion and muscle strength.

• Encourage to express his feelings about his sickness

• Encourage to remember his family, relatives, friends and other memories he had.
