of IV admixtures including antibiotics, hydration, pain medications,

chemotherapy and TPN. Double checking orders, specifically chemotherapy and

TPN including pediatrics. Programming IV pumps (CADD Prizm, Curlin and

Baxter syringe models). Checking IV supplies in the warehouse prior to

delivery and communicating with patients re: inventory needs.

Staff Clinical Pharmacist, New England Home Therapies, February 2011 -

November 2012. Intake pharmacist, filling new orders and medication

profiles in CPR+ software system. Preparation and dispensing of various IV

admixtures including chemotherapy and TPN. Pediatric compounding. C-II

inventory/C-II waste reporting/disposal. IV pumps: CADD Prizm, BodyGuard,

Gemstar, Flo-Gard, Baxter syringe pump. Recommended double checking process
