Lead facilitator for the Intake Department with oversight for smooth operations. Act as a Clinical Liaison to hospitals, as

needed. Work closely with executive management to ensure Hospice guidelines are met and within compliance. At the

front lines of the referral process to educate the community on hospice and home health care services.
