2012 - 2013

120 bed long term care and rehabilitation and skilled nursing home facility including: short and long term

stays - coordinate day to day operations of nursing department, manage budgets, ppd/staffing,

preparation and coordination of PI/QA programs, oversee wound care program, in-services, pharmacy

review, lab protocols, - interview and hire new employees, orientation of new nursing staff - 123 full and

part time employees

Direct and supervise all standards of care provided to ensure the most cost effective services that will

deliver quality patient care. Closely monitor all company process for efficacy, develop action plans when

changes need to be implemented to improve clinical services based on goals and outcomes. Intake

approval and negotiations with private and state insurance companies.

Director of Nursing - The Village at Richardson

2010-2012

280 bed skilled rehabilitation facility which includes: short and long-term stays with an onsite dialysis

unit. current supervise 175 employees comprised of nursing staff, case-managers, social service directors,

nursing assessment coordinators, Admission managers and the rehabilitation therapists.

Coordinate the day to day operation of the facility which includes: prior authorization of high cost

medications, preparation and coordination of our PI/QA programs, oversee the outcomes of our wound

care program, in-services, pharmacy review, lab protocols, orientation of new nursing staff, and all other

aspects of the nursing department.

2003-2010

161 bed long term care skilled rehab. with a 16 bed LTAC. Hired to assume the duties of supervisor of the

nursing department and to implement the Medicare program. Oversaw the day to day operations of

resident care, MD visits, family meetings, staff in-services, hiring with orientation of new employees,

termination of employees when necessary, quality assurance program held monthly, made rounds

routinely with MD's and any other services required to ensure quality resident care.

In charge of all aspects of the Quality Assurance program - included but not limited to: review of the

facility quality indicator reports with changes in protocol to improve the outcomes of quality care.

Responsible for managing the cost of medical supplies, medications and special equipment needed for

acute care patients on skilled wing and patients receiving care on LTAC unit. Monitored the needs of

patients requiring IV, respiratory and ventilator therapy. Directed daily standards of care meetings to

ensure all services were cost effective and partnered with the highest level of care

1998-2003

Managed all duties as assigned per the state regulations and the policies and procedures of the facility.

Resigned due to the company being sold. My administrator was moving to Texas, so I accepted the ffer at

the Oakwoods.

1994-1998

Long Term Care facility - supervisor of nursing personnel, scheduling of patient visits, admission and

discharge visits, hiring and termination of employees, in-services, orientation programs, developes and

implemented the prospective pay program. Supervised approximately 125 aides and nurses during the

time – home health care was attached to this facility

This company was owned and operated by the Rayfords in Alexandria, La - long-term care division

During this time, I worked at multiple facilites which included: Chateau D'arbonne, Lakeview Nursing

Home, Cottage Health Care, Ridgecrest Nursing Home and a 3 month contract at The Oaks Nursing

Home. I was employed by Mr. Rayford as his corporate FN DON, however, he contrated me out when

facilities needed a director of Nursing short or long term, based on the needs of the facility. After Mr.

Rayford passed away, I wanted to try another type of geriatric nursing and went into home health.

included holding monthly meetings to discuss recent surveys, outcomes, and brainstorm for facility

improvements and excellence in resident care.
