Position Duties:

Current employing facility is a transitional, rehabilitation focused SNF of 116 beds, with an attached SubAcute Unit. Completed MDS assessments and associated CAA’s, developed, compiled, wrote, and completed the initial Comprehensive Care Plans, and coordinated and lead the associated new admissions Rounds. Successfully managed the Medicare/PPS process with no late completed assessments, no default dates, and all PPS assessments completed at maximum reimbursement rates possible. Only one state survey completed while here, but the survey had no MDS or care plan deficiencies; for the first time in many years.
