Provide primary care and internal medicine to patients of all ages in a

clinical setting and inpatient care at Life Care and Nash General Hospital.

Provide a wide scope of excellent customer service to clients through

comprehensive health history taking, physical assessment, diagnostic

studies, diagnosis, prescribing medications, chronic disease management,

geriatric care, referrals to specialist, pre and post counseling and

emphasizing on educational awareness, disease prevention including health

promotion.

EXPERIENCE

Hospitalist, Allied Health Professional (AHP); Manchester Memorial

Hospital: April 2012 - Sept 2013.

Provide care to patients of all ages admitted in the hospital with acute

illness through operationalizing clinical frame work that is supported by

evidence-based practice. Responsible for H & P, discharge summaries and

daily rounds with inpatient team case managers. Provide emergency room

analysis through triage of acutely ill patients, and authorizing entire

tertiary admission along with transfers. Providing acute care to

inpatients during hospital stay until the day of discharge. Coordinating

patient care between specialists and arranging specialty consultation care

and/or therapy. Interact clearly and fully with physicians and varied

professional staff related to inpatient care issues. Coordinate inpatient

care, including all diagnostic tests such as X-rays, lab tests or other

procedures. Collaborate effectual delivery of timely and apt medical care

and services. Educating patients and their family and to support every

member of hospital team as required.

Primary Care Provider FNP: ACT Medical Group P. A. March 2011 to October

Providing primary care to designated Assisted Living and Nursing Homes

setup as clinical sites in the underserved counties around Winston-

Salem/Greensboro areas through comprehensive health history taking,

physical assessment, prescribing medications and treatment, therapeutic

management of clients with acute and chronic illnesses. Also counseling,

referrals to specialist consults, ordering and evaluating labs, responding

to triage team and giving orders

coordinating and managing all primary care activities which include

admissions and discharge documentation, signing clients FL2 forms, coding

and billing for provided service. Patient and family counseling including
