review daily documentation, and place the appropriate companion on companion assignments
based on their skill set.
Case Manager: work with up to 19 clients per week, document progress notes, and prepared
initial case management intake paperwork. Work with clients on weekly, short term and long term
goals. Assist with resume building, job search, education enrollment, and interview preparation.
Provide support for accountability, developing structure, and encouragement for wellness. Run
groups such as creative writing groups, building a solid foundation, expressive art, goal setting,
and recovery based groups. Provide weekly communication with family members and treatment
team members working with my clients. Prepare treatment contracts when needed. Compile
information and resources for transition from the CAST program and complete discharge
planning and final discharge plan. Network with potential referents and provide status update to
