Retired on August 15, 2008 and went back immediately full time in my
previous position as Records Coordinator for University Counseling
Services. Returning after retiring is permitted for a set number of hours
per year. I used up the majority of these hours working full time for this

Sept. 2000 - Aug 15, 2008. CSUN - University Counseling Services, Records
Coordinator.
University Counseling Services provides psychological and psychiatric
services to all students attending California State University, Northridge.
As records coordinator I was in charge of all psychiatric and psychological
records for the students. I set up the charts, kept tract of visits, notes,
and maintained them in accordance with HIPPA regulations. I do have a HIPPA
certification. I assisted in scheduling appointments for the various
counselors and psychiatrists. There were a total of 14 counselors, 2 staff
psychiatrists, 4-5 fourth year psychiatric residents from the VA, and 4-5
psychology interns completing their 4th year. I was in charge of all
records and the maintaining thereof.
I ordered the labs requested by the psychiatrists and went on line to
pull the results and then gave them to the ordering psychiatrist, after
being reviewed these were then placed in the clients file. I also set up
the patient's assistance program through the various drug companies to
obtain free medication for students with no insurance and low income. I
filed all paperwork required and checked eligibility for these programs. I
then received the program medication which was given to the psychiatrist to
give to their client. I also handled receipt of sample medications and kept
tract of them. These were made available to the psychiatrists for their
clients.
Also my responsibility with the records was to maintain that they were
confidential. These were kept in locked file cabinets when not in use by
the counselors or psychiatrists. They were pulled every day and given to
the appropriate person when an appointment was scheduled. The files for
previous years were kept locked up. Files were kept by law for a period of
8 years and then if there was no activity they were shredded.
I also received requests for records from individuals and courts.
These requests were given to the appropriate counselor for review and if
deemed permissible, the records were copied and sent to the requesting
party. Only copies were sent, no originals were sent unless under very
special circumstances, and then only with the permission of the counselor
or the chairman of the department.

March 2000 - Sept. 2000: CSUN - University Parking Services
I was hired as an emergency person to fill an open position within
the University Parking Services. In this position, I received parking
violation fees, imputed parking tickets into the system and coordinated
monies coming in from the various parking booths and meters.
July 1999 - March 2000: Hammond & Hammond Property Management.
I was in charge of making new property management files and
maintaining those already in existence. I also answered phones and did all
computer work as necessary, including running credit reports on prospective
renters for the property owners.
1993 - June 1999: Las Virgenes Unified School District:
Served in various office positions from health clerk to office
manager at the various schools within the district. Spent the last 3 years
at the district office. In this capacity I work in all areas from personnel
to payroll to accounting and purchasing. In payroll I imputed monthly
salaries for the various personnel within the district, mainly these were
for substitutes and sometimes they held up to 7 or 8 different positions
and these had to be separated as to different accounts and pay rates. I
worked in purchasing and also accounts receivable.
January 1973 - 1993
Homemaker and mother. Volunteered at the schools and assisted in
various sports programs. Raised and assisted with all variety of domestic
animals. Health issues and training.
July 1965 - Jan 1973: UCLA research associate, Department of Anatomy
Was the lab research associate for a nationally known Neuro-
endocrinologist. Research was on the study of the brain and behavior. I was
in charge is all surgery and equipment for the work. There were many post-
doctoral fellows from around the world that came here to learn various
techniques. Also researchers from various countries came to do research.
Education: B.S. Animal Science from University of California - Davis
Graduated June 1965
Presently a volunteer with Wildlife Care of Ventura, helping in transport
of all wildlife and raptors in the North Valley and Santa Clarita area,
plus picking up animals and birds from various L.A. City Animal Control
Shelters.
