Developmental Disabilities Service Coordinator

Perform initial contact with individuals wanting Service Coordination Services.

Aid in loading Relative/Kinship Packets from the placement email box using Microsoft Outlook to create Foster Care Organizations in N-Focus. Ensure all information including household demographics are received to create an accurate Foster Care Organization. Work with external agencies to ensure incorrect information is corrected in a timely manner. Create the Relative/Kinship Foster Care Organization record using N-Focus to include creating a license, adding services approvals, facility type and assignment of the Organization. Send emails after the completion of the Organization using Microsoft Outlook. Receive completed home studies for Relative/Kinship Foster Care Organization from NFC. Compile the data and send them to the appropriate Resource Development worker as found on the official Eastern Service Area tracking sheet using Microsoft Excel. Aid in creating new Service Organizations to help set up payments for Providers who have provided services for children in the Foster Care System using N-Focus and W-9 documentation. Created the excel spreadsheet of file tracking when Resource Development files are checked out/sent for an Audit. Created the excel spreadsheet of Adoption/Guardian Subsidies sent to IMFC. Created an excel spreadsheet to record all Background Check Requests which are sent. Spreadsheet includes missing information tabs, completed releases, and expired releases for the year. Responsible for completing Full Background Check Requests sent in by Child Placing Agencies for: Resource Development Unit, Adoption Unit, and Placement of Probation Youth. Complete an Adoption Found/Unfound letter for Foster Care Organizations adopting children in the Foster Care System. Responsible for ensuring Background Check Request forms have correct information and/or not missing any fields. Research who currently resides in the home according to N-Focus and alerting Child Placing Agency of any needed releases. When incorrect information is given, talk with the Child Placing Agency Representative regarding missing or needed fields. Provide duplicate background check results to Child Placing Agencies when requested. Created FBI/State Patrol spreadsheet to record all FBI results in the ESA that has been sent to Central Office. Document all FBI/State Patrol results on the spreadsheet, put each result in a folder, label each folder and packed results to be sent to Central Office. Responsible for inventory and removing FBI and State Patrol results from all files currently in the Resource Development file room and from all files sent to Closed Records. After removing all FBI/State Patrol results, confirm if duplicate files have already been sent to Central Office. If duplicates are in Central Office, responsible for packing FBI results, labeling which box the files are located in and send to worker located at Central Office. Created Resource Development Excel spreadsheet to reflect all Resource Development files sent to Closed Records. Spreadsheet includes Document Organization Name, Organization Id, Organization SSN, Close Date of Organization, if Service Approvals are open or closed and any other Comments. Separate pages for multiple files, active files, currently closed Relative/Kinship files for the year and currently closed foster home files for the year. Created excel tracking spreadsheet for audit files leaving Resource Development Secure File room and when the files are returned. Experience in Guardianship and Adoption Subsidies inputs into N-Focus. Creating copies of Guardianship and Adoption Subsidies for IMFC and Adoption worker. Contacting NFC worker to have the date changed when a Subsidy date is entered incorrectly. Send original Subsidy paperwork to Adoptive/Guardianship parent. Formerly responsible for creating Relinquishment folders for the Adoption Unit and NFC. Send signed Relinquishment letter to Douglas or Sarpy County Attorney and/or receptionist. Responsible for gathering data for monthly tracking of background check request that were received, completed, withdrawn, completed for other Service Areas. This includes the number of times a release was required to be sent back to an Agency due to missing information. Responsible for maintaining accurate inventory and cleanliness of the Resource Development Secure Record File room including proper filling of large amounts of records and boxes. Use alphanumeric procedures to assure accountability and retrieval as needed. Send boxes of records for retention and/or destruction as requested or as mandated by State and Federal regulations. Responsible for shipping and receiving large volumes of records and boxes of record storage, maintaining inventory levels and needed supplies of the warehousing of said materials. Request needed supplies for inventorying Resource Development, Adoption and Fingerprint Files.
