Responsibilities

I have worked with individuals that are diagnosed with Autism, Cerebral Palsy (Spastic Quadriparesis),

Acrodysostosis, Unspecified Intellectual Disabilities, Mild Cranial Facial Dysplasia, as well non-verbal clients.

Assisted consumers who are disabled or chronically ill with their activities of daily living (ADLs) whether

within the home, outside of the home, or both.

Assisted clients with personal, physical mobility and therapeutic care needs, usually as per care plans

established by a rehabilitation health practitioner or health care professional.

Facilitate the clients’ departure and arrival for work or daily community integration activities.

Instruct client in proper behavior in the community.

Have all reports (Daily Activity Log, Task Analysis, etc…) documented and turned in when due.

Participate in consumer assessment, planning, and evaluation processes.

Report special incidents to supervisor and suspected resident abuse according to applicable laws and
