emphasize some of my administrative, clinical and academic skills. In

August 2000 I joined University of Illinois College of Medicine at Peoria

(UICOMP) as a faculty member and have been serving various academic,

clinical, and administrative roles. I am currently an Associate professor

of Clinical Medicine and Section Chief of Infectious diseases at UICOMP.

Being a Masters in Public Health (MPH) in community health sciences I could

provide several educational and clinical expertise to the local health

department and the community organizations. I am pleased to mention that I

was selected as one of the lead physician for strategic planning and in the

implementation of hospital system wide Electronic Medical Record (EMR). In

2010, we successfully launched EMR incorporated with Computerized Provider

Order Entry (CPOE) along with safety and quality measures for clinical

documentation. In addition, I have played a key role in further EMR

expansion and interfacing of Antimicrobial Stewardship Program (ASP);

evidence based clinical service on appropriate utilization of antibiotics.

The ASP resulted significant savings to the institution. I am proud to

mention that UICOMP infectious diseases section members are the primary

providers for HIV/AIDS services in the central Illinois. With the new

developments and trends in the health care system we are actively

transitioning into Patient Center Medical Home (PCMH) model.

As outlined in my CV, my leadership, teamwork, and collaborative abilities

in academia and clinical skills have been recognized through several

teaching excellence awards and outstanding teacher awards. I am recipient

of 2005 & 2010 "UIC Teaching Recognition Award". It is a prestigious honor

awarded to twenty selected members from the UIC four campuses among 5000

Accomplishments as Chief of Section:

Research Project: QuantiFERON-TB Gold Test (Cellestis): It is a

quality and reliability analysis research, which I spearheaded on

yearly tuberculosis (TB) screening test. This project involved more

than 6000 healthcare workers of the organization; the study changed

SFMC TB screening policy. This resulted in significant financial gain

to the organization. The study titled "Questionable effectiveness of

the QuantiFERON-TB Gold Test (Cellestis) as a screening tool in

healthcare workers" (http://www.ncbi.nlm.nih.gov/pubmed/20979495 )

Education/Research: Reviewed and redesigned medical students and

resident's clinical rotation with incorporation of online pre and post

tests, Team Based Learning (TBL) etc. Additionally, by increasing the

scope and breath of clinical rotation with the curriculum to include

the global and international medicine.

Evidence Based Project: Antimicrobial Stewardship Program (ASP): This

clinical service is strategically planned, structured and implemented

in SFMC in Oct 2011. The ASP is an innovative and intensive practice-

based multi-disciplinary approach for the appropriate use of

antimicrobial agents. The ASP positively impacted specific to patient

to quality and safety, while providing financial gain to the medical

center. At present we are in the process of expanding ASP services to

OSF Health Care System associated ambulatory clinics and eight

satellite medical centers.

(B) Medical Director OSF Infectious Disease Out-Patient Clinic, 2007-

Present

. Oversee and provide direction to clinical operations, patient

referrals and evaluation.

. Developing, approving, and updating protocols and practice

bundles.

. On-going quality improvement, patient safety and goal

development.

. Strategies to facilitate compliance with reimbursement

regulations.

. Periodic evaluation and feedback individual service program.

. Quarterly evaluation and updates on EMR to meet provider &

service requirements.

. Liaison between the infectious disease service, other staff

Accomplishments as Medical Director OSF Out-Patient Clinic:

Expansion of Services-Cystic Fibrosis, Travel and Hepatitis Clinic:

With progressive planning and additional recruitment, the ambulatory

services are expanded to include wound care and home health care.

Further expansion is planned for travel clinic, multi-disciplinary

approach to cystic fibroses population in conjunction with pulmonary

and hepatitis clinic with gastroenterology service.

(C) Chair Infection Control OSF- St. Francis Hospital & Medical

Center-2003-2007

. Advisory member for hospital sentinel committee.

. Modified and implemented CDC isolation protocols for MRSA,

VRE, and Clostridium difficile.

. Developed policies and protocols for Multi Drug Resistant
