My name is Sarah Slack. I am currently enrolled in the Family Nurse Practitioner program at the

Massachusetts College of Pharmacy and am seeking a position at your facility. I am graduating and will be

board eligible on May 10, 2014.

I possess strong nursing experience in the ICU and have fulfilled both didactic and clinical requirements to

obtain my family nurse practitioner license. I have been exposed to many people with varying diagnosed, and

have been challenged to face many critical situations which have taught me valuable skills. My ability to

multitask, recognize critically ill patients, and adapt to a quickly changing environment have enhanced my

critical thinking skills. Also, I have had thorough training as a student nurse practitioner in a variety of

practices, focusing on the entire lifespan. I am extremely hard working and quick to learn in new

As a student NP, I assisted in providing care in both normal and high risk pregnancies with

on-site non-stress testing, ultrasounds and amniocentesis. Gynecological care included

routine exams, pre-conception counseling, pap-smear evaluation and treatment, STD

screening, birth control counseling. This practice also had onsite laboratory,

mammography, x-rays, and ultrasounds available. I was fortunate to participate in office

procedures, including IUD insertion and colposcopy’s.

Experience Staff Nurse in the Mixed Intensive Care Unit (MICU)

As an RN in the MICU, I have used my advanced skills to care for patients who are

critically ill and are at high risk for life-threatening health problems. I have administered

vasoactive medications, sedation, narcotic drips for pain control, blood products, and

provided wound care. I have assisted physicians in bedside procedures, including rapid

sequence intubation, central line insertion, bronchoscopy, PEG tube insertion, tracheotomy

insertion, and chest tube insertion. I have learned to utilize the technology common in the

ICU, including ventilators, ICP monitoring, arterial lines, and CVP monitoring. I have

learned to be organized and multitask in a very busy setting, as well as make quick

decisions with a rapidly deteriorating patient. I have continued to form therapeutic

relationships with patients and their families, educate on the disease and healing process,

perform full and detailed assessments on pre-op and post-op patients, interpreted EKG’s

and cardiac monitor rhythm strips, obtained lab specimens from PICC lines and Triple

Lumen Catheters, inserted intravenous catheters, transfused blood products, interpreted lab

results, inserted Foley catheters and fecal management systems, provided comfort and end

of life care, and performed meticulous wound care including dressing changes and

debridement.

Staff Nurse on a Medical-Surgical/Telemetry Unit

As an RN in an acute care facility on a medical-surgical/telemetry unit, I learned basic

nursing skills including evaluating, assessing, preparing and implementing treatment plans

for sick or injured patients providing the highest level of patient care. Other essential duties

and responsibilities included carrying out physician orders, administering prescribed

medications, applying dressings, and monitoring vital signs. I would care for up to three

telemetry patients in a shift, monitoring EKG’s and interpreting other data from cardiac

monitors.
