Duties: Making home care visits primarily to patients homes after discharge from hospital. Care needs for
many different types of wounds and doing in depth health assessments. Travel to many different areas of
Northern,Va. Perform blood sugar testing, PT/INR monitoring,wound cultures and blood drawing.Prepare
medication cassettes for those adults in the home setting who have difficulty seeing. Ostomy care.Supra

concerns of 67 residents and families within memory impaired and assisted living units.
Tasks also include communicating patient needs and laboratory analysis results to psychiatrist and
medical doctors (MDs). In addition, schedules required psychiatrist and MD visits. Supervises activities of
CNA and AMAs.
