January 2014-current: RN Case Manager for homecare manager of patient

living out in the community either alone or with a caregiver. I am

responsible for OASIS documentation to open assessments, to bring them

under our care and to assess them weekly. Education patient and care givers

and primary focus was to make sure my participants were safe living in the

community. I did home safety checks every 6 months. I performed semiannual

was delivered to them, as far as home health care, medications and

education in the home for caregivers or participants. I did med boxes if

Job Description: I am currently responsible for staffing of the

clinic. I oversee the care of 303 participants and there care at 2 clinic

sites. I have a Eastside and Westside clinic to staff and oversee the

function and well being of the care provided to the participants. I used

also very hands on with blood draws, patient care, wound care and

Job Description: I was hired as admissions and wound care nurse and

performed that task for approximately 4 months. I was responsible for

weekly daily and weekly wound care for the entire facility wounds. I

measured and documented wounds weekly and responsible for reporting to the

doctors and NP the decline or progression of all wounds in facility. I

I performed my nursing duties as a Floor nurse working on the Rehab unit. I
